Abstract

Background: The aim of nursing care is "to allow the patient to maintain his/her life under the conditions closest to the daily life of him/her, to take patient relieving measures to prevent the possible complications and to help him/her to become self-sufficient in his/her physical, psychological and social life as soon as possible". Objective: In this study, it was aimed to investigate the effects of discharge education given after total hip replacement surgeryon the daily life activities and quality of life of the patients. Method: The sample of the quasi-experimental study consisted of 50 patients, with 25 of them being the experimental group and 25 being the control group. The control group did not receive any education and a general procedure of the orthopedics clinic was applied. At months 1 and 3 after discharge a visit for the patients in the control group was performed, and Katz Index of Independence in Activities of Daily Living and SF-36 Quality of Life Scale was applied again. A descriptive form was filled by the patients in the control group before the surgery and the patients were applied Katz Index of Independence in Activities of Daily Living and SF-36 Quality of Life Scale. The patients in the experimental group received the discharge education on the 3rd day after total hip replacement surgery and the training manual prepared was given to the patients after the education process. At months 1 and 3 after discharge a visit for the patients in the control group was performed, and Katz Index of Independence in Activities of Daily Living and SF-36 Quality of Life Scale was applied again. Results: For daily living activities in the study, there was no difference between the two groups before surgery (p> .05), but there was a significant difference in the first and third months (p < .001). Again, there was no difference between the two groups for the scores obtained from the sub-components of SF-36 scale before surgery (p > .05), whereas there was found a significant difference between the sub-components like physical function, physical role, pain, vitality, emotional role and mental health in the experimental group at the end of the third month (p < .001). Conclusion: It has been determined that there is an increase in score averages of SF-36 scale and in score averages of Katz Index of Independence in Activities of Daily Living in the experimental group with the discharge education given after the total hip replacement surgery, and these patients perform their daily life activities more independently.

Özet

Giriş: Hemşirelik bakımının temel amacı; “hastanın günlük yaşam biçimine en yakın koşullarda yaşamını sürdürmesi, oluşabilecek komplikasyonları önlemeye yönelik hastayı rahatlatıcı önlemlerin alınması ve fiziksel, psikolojik ve sosyal yaşamında mümkün olan en kısa zamanda kendi kendine yeterli duruma gelmesine yardımcı olmaktır”. Amaç: Bu çalışmada, total kalça protezi ameliyatı sonrası verilen taburculuk eğitiminin hastaların günlük yaşam aktivitelerine ve yaşam kalitesine etkisinin incelenmesi amaçlanmıştır. Yöntem: Yarı deneysel türdeki çalışmanın örneklemini 25 deney 25 kontrol grubu olmak üzere 50 hasta oluşturmuştur. Kontrol grubu hastalarına herhangi bir eğitim verilmemiş ve taburcu olduktan sonraki 1. ve 3.aylarda kontrol grubu hastalarına ev ziyareti yapılarak, Katz günlük yaşam aktiviteleri ölçeği ve SF-36 yaşam kalitesi ölçeği tekrar uygulanmıştır. Deney grubu hastalarına ameliyattan önce hazırlanan Hasta Tanıtım Formu doldurulmuş, Katz günlük yaşam aktiviteleri ölçeği ve SF- 36 yaşam kalitesi ölçeği uygulanmıştır. Deney grubu hastalarına total kalça protezi ameliyatından sonraki 3.günde taburculuk eğitimi verilmiş ve hazırlanan eğitim kitapçığı, eğitim sonrası hastalara tekrar yararlanmaları için teslim edilmiştir. Taburcu olduktan sonraki 1. ve 3.aylarda kontrol grubu hastalarına ev ziyareti yapılarak, Katz günlük yaşam aktiviteleri ölçeği ve SF-36 yaşam kalitesi ölçeği tekrar uygulanmıştır. Bulgular: Çalışmada günlük yaşam aktiviteleri açısından ameliyat öncesinde iki grup arasında fark bulunmazken (p > .05), birinci ve üçüncü aylarda ileri derecede anlamlı fark bulunmuştur (p < .001). Yine ameliyat öncesinde SF-36 ölçeği alt bileşenlerinden alınan puanlar açısından iki grup arasında fark bulunmazken (p > .05), üçüncü ayın sonunda deney grubu hastalarında fiziksel fonksiyon, fiziksel rol, ağrı, vitalite, emosyonel rol ve mental sağlık alt bileşenlerinde ileri derecede anlamlı fark bulunmuştur (p < .001). Sonuç: Total kalça protezi ameliyatı sonrasında verilen taburculuk eğitimi ile deney grubu hastalarının SF-36 ölçeği puan ortalamasında ve Katz GYA ölçeği puan ortalamasında artış olduğu, günlük yaşam aktivitelerini daha bağımsız yerine getirdiği belirlenmiştir.
