Abstract

Objective: We aimed to investigate the association between hand functional level and bone mineral density of wrist in patients with hemiplegia after stroke. Materials and Methods: 20 patients with hemiplegia that occured due to cerebrovascular accident, were enrolled into the study. The mean age of the patients was 61,24±3,29 years. Brunnstrom stages were used to evaluate the motor recovery and Ashworth Scale was used for spasticity. The assessment of the grip strength of affected and unaffected side was performed with Jamar Dynamometer. The hand dexterity was evaluated with Jebsen hand function test and daily living activities with Barthel index. Serum PTH, 25-OH Vit-D and urine deoxypyridinoline level were determined, additionally routine biochemical analysis and complete hemogram. DEXA were used for bone mineral density (BMD) measurements . Results: The wrist BMD on the affected side was significantly lower than the unaffected side. The significant correlations were established between the wrist BMD and age, brunnstrom stages of upper extremity and hand, spasticity level of hand, grip strength on the affected side, Jebsen hand function test, Barthel index and 25-OH Vit- D level. In regression analysis, "the functional activity level of the hand" was found as most important determinant of the wrist BMD on the affected side in patients with hemiplegia after stroke (β=4.622, p=0.001). Conclusion: We think that the interventions for improvement of hand functions should be take placed in routine neurologic rehabilitation program to avoid fractures and morbidity due to bone loss in wrist in patients with hemiplegia after stroke.

Her iki el bileği kemik mineral yoğunluğu (KMY) ölçümlerinde DEXA (Lunar DPX-IQ) kullanıldı. Bulgular: Yapılan ölçümlerde hastaların etkilenen taraf el bileği KMY değerleri, sağlam tarafa göre istatistiksel anlamlı olarak düşüktü. Korelasyon analizinde yaş, üst ekstremite ve el brunnstrom evresi, elde spastisite düzeyi, etkilenen taraf kavrama gücü, Jebsen el fonksiyon testi, Barthel indeksi ve 25-OH Vit-D düzeyi ile el bileği KMY değerleri arasında istatistiksel olarak anlamlı korelasyon saptandı. Yapılan regresyon analizi sonucunda ise, inme sonrası hemipleji gelişen hastalarda etkilenen taraf el bileği KMY değerinin en önemli belirleyicisinin, elin fonksiyonel aktivite düzeyi olduğu tespit edildi (β=4.622, p=0.001). Sonuç: İnme sonrası hemipleji gelişen hastalarda, el bileğinde KMY deki azalmaya bağlı kırıkların ve buna bağlı morbiditenin artmaması ve kendine bakım aktivitelerinde bağımsızlık düzeyinin yükselmesi için, elde ince fonksiyonel aktivitelerin gelişimini sağlayacak girişimlerin, rutin nörolojik rehabilitasyon programı içerisinde erken dönemden itibaren yer almasının önemli olacağı kanısındayız.
