Abstract

Objective: Group A beta-hemolytic streptococci (GAS) can cause a wide range of diseases, including mild illnesses such as uncomplicated tonsillo-pharyngitis or severe illnesses such as life-threatening systemic infections. Penicillin is the first drug of choice for treatment of GAS infections. Macrolides can be used in management of patients with penicillin allergy. However there is concern for an increasing resistance to macrolides. The aim of this study was to determine asymptomatic carriage rate of GAS in children of primary school age in Konya, and to investigate the antimicrobial susceptibilities of GAS isolates. Material and Methods: Throat swabs taken from 509 children, ranging in age from six to fourteen, were analyzed for GAS in Medical Microbiology Laboratory at Selcuk University Faculty of Medicine. Results: Carriage rate of GAS in school children was 7%. All GAS isolates were susceptible to penicillin, erythromycin, clindamycin, azithromycin, ceftriaxone, levofloxacin, chloramphenicol, vancomycin, and linezolid. Twelve isolates were intermediate (33%), and four were resistant (11%) to tetracycline. Conclusion: : In this study, it was determined that carrige rates in children were in expected ranges. Macrolides can be preferred in treatment of GAS infections in case penicillin can not be used.

Özet

Amaç: A Grubu Beta Hemolitik Streptokoklar (AGBHS), komplike olmamış tonsillofarenjit infeksiyonlarından hayatı tehdit eden sistemik infeksiyonlara kadar geniş yelpazede pek çok hastalığa neden olabilmektedir. AGBHS infeksiyonlarının tedavisinde ilk tercih edilecek ilaç penisilindir. Penisiline allerjisi olan hastalarda makrolidler kullanılabilir. Ancak makrolidlere direnç giderek artmaktadır. Bu çalışmanın amacı, Konya ilinde ilköğretim çağındaki çocuklarda AGBHS taşıyıcılık oranının belirlenmesi ve izole edilen suşların antibiyotik duyarlılıklarının araştırılmasıdır. Gereç ve Yöntem: Yaşları 6-14 arasında değişen 509 çocuktan alınan boğaz sürüntü örnekleri Selçuk Üniversitesi Tıp Fakültesi Tıbbi Mikrobiyoloji Laboratuvarında AGBHS açısından değerlendirilmiştir. Bulgular: AGBHS taşıyıcılık oranı %7 olarak saptanmıştır. İzole edilen bütün suşlar penisilin, eritromisin, klindamisin, azitromisin, seftriakson, levofloksasin, kloramfenikol, vankomisin ve linezolide duyarlı bulunmuştur. On iki AGBHS izolatında tetrasikline orta duyarlılık (%33) ve 4'ünde ise direnç (%11) tespit edilmiştir. Sonuç: Çalışmamızda taşıyıcılık oranlarının beklenen düzeylerde olduğu belirlenmiştir. Makrolidlerin, penisilinin kullanılamadığı durumlarda AGBHS infeksiyonlarının tedavisinde tercih edilebileceği ortaya konmuştur.
