Abstract

Objective: The aim of this study was to compare the outcomes of rehabilitation programs based on either an immobilization or early mobilization protocol after repair of hand nerve injuries; outcomes were assessed using physical, functional and electrophysiological methods. Materials and Methods: Thirty patients (7 females [23.3%], 23 males [76.7%]; mean age: 28.6±10.6 years) were included in the study. After nerve repair Group 1 (n=16) patients were rehabilitated with an immobilization protocol (3-week immobilization by splint), while Group 2 (n=14) patients were rehabilitated with early mobilization protocol (mobilization in 1-3 days). Evaluations were made superficial sensorial evaluation with Semmes Weinstein monofilament test, cortical sensation, grip strength, lateral strength, daily living activities and nerve functional level at the initial visit and 24th week. Electrophysiological evaluations were made at the 4th and 24th weeks. Results: Grip strength, lateral strength and daily living activities showed significant improvement in both groups at the 24th week but according to group comparisons Group 1 was superior to Group 2. Nerve functional level and electroneuromyographic evaluation results were also in favor of Group 1 (p=0.001, p=0.003 respectively). Conclusion: Following repair of hand nerve injuries, although rehabilitation with immobilization or early mobilization protocols produced satisfactory results both physically and functionally, immobilization demonstrates superiority compared to early mobilization with respect to nerve functional level and electrophysiological improvement.

Bulgular: Her iki grup 24.haftada kavrama gücü, lateral kavrama gücü ve günlük yaşam aktiviteleri bakımından anlamlı gelişme gösterirken gruplar arası karşılaştırmada Grup 1 Grup 2'ye üstünlük gösterdi. Sinirin fonksiyonel seviyesi ve elektronöromiyografik değerlendirmeler yine Grup 1 lehine sonuçlandı (sırasıyla, p=0.001, p=0.003). Sonuç: Sinir yaralanmalarının rehabilitasyonunda immobilizasyon ve erken mobilizasyon yöntemleri fiziksel ve fonksiyonel sonuçları bakımından memnuniyet verici olsa da sinirin fonksiyonel seviyesi ve elektrofizyolojik iyileşmesi göz önünde bulundurulduğunda immobilizasyonun erken mobilizasyona üstünlük sağladığı belirlenmiştir.
