Abstract

Objective: The aim of the present study was to compare the effects of two different viscosupplementation agents for the functionality and pain of patients whom had a surgical indication were in the fourth grade of Kellgren-Lawrence radiological criteria's and did not accept the surgery for knee osteoarthritis. Methods: Sixty-eight patients having the clinical diagnosis of knee osteoarthritis according to ACR criteria's were included in the study. All patients were in the fourth grade of Kellgren-Lawrence radiological criteria's, had an indication of surgery unless all were rejected the surgery. Patients were divided in to two groups by similar age and gender; 1. Group, Hylan-GF 20 (16 mg/2 ml) was injected once a week for 3 times, 2. Group 15 mg/2 ml Sodium-Hyaluronate was injected for the same time intervals to 34 patients. The clinical evaluations of patients were done according to the Lysholm knee scale on the first day of treatment and after 24 weeks. Results: The beginning mean clinical score of patients according to Lysholm knee score in group 1 was increased from 55.6 to 79.9 on the 24 week of treatment with Hylan-GF-20 in which group 2 scores were also increased from 56.2 to 80.1 with Sodium-Hyaluronate. When all patients were evaluated, good results were obtained for the %97 of them. Conclusion: The viscosupplementation therapy with hyaluronic acid and its cross-linked derivatives is efficient and trustable in symptomatic therapy of knee osteoarthritis during 2 to 3 degrees. In this study, the treatment done to the patients whom were rejected surgery, showed us the wellness of patients and conclude that the need for total knee replacement can be delayed by viscosupplementation.

Özet

Amaç: Bu çalışmanın amacı Kellgren-Lawrence radyolojik evrelemesine göre 4. evrede, cerrahi endikasyonu olan diz osteoartritli hastalardan, cerrahi uygulamayı kabul etmeyenlerde, iki farklı ajanla yapılan viskosuplemantasyonun ağrı ve fonksiyonellik üzerine olan etkisini incelemekti. Yöntemler: American College of Rheumatology kriterlerine göre diz osteoartriti tanısı alan 68 hasta çalışmaya alındı. Bulgular: 16 mg/2 ml Hylan-GF 20 enjeksiyonu uygulanan 1. guruptaki hastaların Lysholm diz skalasına göre ortalama başlangıç değerleri 55,6'dan tedavinin 24. haftasında ortalama 79.9'a yükselirken 15 mg/2 ml sodyum hyaluronat uygulanan 2. gurupta ortalama 56,2'den 80,1'e yükseldi.
