Abstract

Objective:Because pneumonia in stroke patients is often the result of aspiration, systematic use of a dysphagia screen may result in a significantly decreased risk of pneumonia and an improved general outcome. The aim of our study was to evaluate clinical bedside of swallowing function in patients with acute stroke. Methods:Fifty randomize patients with acute stroke who were admitted to Neurology departmentbetween January and December 2010 were included in this study. Degrees of neurologic deficit inpatients were evaluated with NIH (The National Institute of Health) stroke scale. Disability were determined by the Rankin Scale and Barthel Index. Dysphagia score with neurologic examination wasdetermined. Stroke patients were examined with GUSS (Gugging Swallowing Screen) test for quantitative evaluation swallowing function at first 1 week. Results:Fifty patients who had acute cerebrovascular lesion (24 women, 26 men; mean 66.7±14.3yr) wereincluded in our study. According to the GUSS test classification; while 15 patients had moderate- severe dysphagia, 35 patients had mild dysphagia. There were statistically significant negative correlation between NIH stroke scale and GUSS testscores. Also, there was statistically significant negative correlation between GUSS test score and neurological dysphagia evaluation scale score.Functional disability reflected by Barthel score and Rankin score was significantly better in patients without dysphagia than in those with dysphagia. Conclusion:Although videofloroscopic evaluation is the most objective method of determining dysphagia and aspiration risk, bedside clinicalassessments for the evaluation of swallowing remain as rapid, low- cost and reliable methods in patients with stroke.

Özet

Amaç: İnmeli hastalarda aspirasyon sıklıkla pnömoni ile sonuçlandığı için, disfajinin taranması aspirasyon pnömoni riskini önemli derecede azaltabilir ve genel sağlık sonuçlarını düzeltebilir. Çalışmamızın amacı, akut inmelihastalarda, yutma fonksiyonunun yatak başı klinik olarak değerlendirilmesiydi. Metod: S.B. Hastalardaki nörolojik defisit derecesi, NIH (The NationalInstitute of Health Stroke Scale) inme skalası ile değerlendirildi.
