Abstract

Introduction: Delirium is characterized by impaired cognitive functions, changes in consciousness, attention disorders, disturbance in sleep-wake cycle, is a transient organic mental syndrome with sudden onset. In intensive care unit patients, delirium has been reported as 40-80% in the literature. APACHE II and SOFA scoring systems are routinely used in all intensive care units. While APACHE II is a prognostic scoring system, SOFA score is used to describe organ dysfunction. In this study, we aimed to show the frequency of delirium and the relationship between delirium and intensive care scoring systems in patients who were followed up in the first step internal medicine intensive care unit. İzmir Eğitim ve Araştırma Hastanesi Tıp Dergisi (Medical Journal of İzmir Hospital) 24 (1): 29-34, 2020 30 İzmir Eğitim ve Araştırma Hastanesi Tıp Dergisi (Medical Journal of İzmir Hospital) Material and metods: The study included 262 patients who were followed up in 8-bed internal medicine intensive care unit. Patients who were hospitalized and entubated due to primary neurological diseases, trauma, etc. were not included in the study. ICU confusion assessment scale was applied to the patients. Intensive care scoring systems SOFA and APACHE II were used for all the patients. The relationship between delirium and ICU scoring systems was evaluated. Results: Delirium was seen in 51% of the patients. The mean age of the patients was 71± 14,3 years. The mean duration of the hospitalization was 7,09± (6,5) days. The mean SOFA score of patients with delirium was 9,93 ± 2,60 and the group without delirium was 10,48 ± 2,85 (p = 0,035) While the mean APACHE II score of the group with delirium was 22,29± (4,60), the mean score of the group without delirium was 22,97± 4,76 (p = 0,07). Conclusion: Scales routinely used in ICUs (such as APACHE II, SOFA score) are not sufficient to evaluate delirium as delirium is not associated with disease severity. Delirium may also occur frequently in patients with low SOFA and APACHE II scores.

Özet

Giriş: Deliryum ani başlangıçlı, genel olarak bilişsel işlevlerin bozulması, bilinç durumunda değişiklik, dikkat bozuklukları, uyku döngüsünde düzensizlik ile karakterize, geçici organik mental sendromdur. Literatürde yoğun bakım hastalarının %40-80’inde deliryum geliştiği bildirilmiştir. APACHE II (Acute Physiology and Chronic Health Evaluation) ve SOFA (Sequential Organ Failure Assessment Score) skorlamaları günümüzde tüm YBÜ rutinde kullanılan skorlama sistemleridir.
