Abstract

This study presents the diagnostic and therapeutic management of a large odontogenic keratocyst (OKC) closely associated with the mandibular canal using a conservative two-stage treatment approach. In a 17-year-old male patient, a unilocular radiolucent lesion measuring approximately 2×4 cm and extending toward the mandibular ramus was detected in the left mandibular third molar region and was radiologically evaluated with a preliminary diagnosis of OKC. The treatment plan consisted of initial marsupialization followed by enucleation. Histopathological examination confirmed the diagnosis of plaque-type OKC, and a significant reduction in lesion volume was observed during the 12-month follow-up period. Enucleation and extraction of the associated tooth were subsequently performed as the second stage of treatment. No recurrence was observed during the two-year radiographic follow-up. This case demonstrates that a conservative two-stage treatment approach is an effective option for large OKCs involving the mandibular canal, as it reduces the risk of recurrence while minimizing the likelihood of neural injury.

Histopatolojik inceleme plak tip OKC tanısını doğrulamış, on iki aylık takipte lezyon hacminde belirgin küçülme gözlenmiş, ikinci aşamada enükleasyon ve ilgili dişin çekimi gerçekleştirilmiştir. İki yıllık radyografik takipte nüks izlenmemiştir. Bu olgu, mandibular kanal komşuluğu bulunan geniş OKC’lerde konservatif iki aşamalı tedavi yaklaşımının, nüks riskini azaltırken sinir hasarı olasılığını da en aza indiren etkili bir seçenek olduğunu göstermektedir.
