Abstract

Aim: Alpha blockers have been shown to be effective in treating various types of neuropathic voiding dysfunctions in children. In this study we investigated the effectiveness of treatment with alpha adrenergic blockade in children with residuel urine due to detrusor sphincter dyssynergia. Material and Methods: The alpha blocker doxazosin (0.5 to 2.0 mg daily) was administered to 22 children (5 to 13 years). Children were treated with alpha-adrenergic blocker Doxazosin for at least 6 months. Children had various etiologies, including myelomeningocele, sacral agenesis, anal atresia and nonneurogenic neurogenic bladder. Urodynamics and postvoid residual volumes were checked before and 6 months after doxazosin treatment. Results: Complete and partial improvement of incontinence was noted in 11 and 4 patients, respectively. Improvement of maximal flow rate and decrease in leak point pressures were noted in 13 and 15 patients respectively. Reduction in the postvoid residuel urine was noted in 13 patients. No significant adverse effect was seen during treatment other than mild decreases of systolic and diastolic blood pressure which was seen in two patients. Conclusion: Alpha blocker doxazosin is effective in the treatment of children with residuel urine due to detrusor sphincter dyssynergia.
