Abstract

Aim: Retrospective study was performed to evaluate risk factors, clinical features and treatment modalities of portal vein thrombosis (PVT) after splenectomy (SPT) in pediat-ric hematologic diseases. Material and Methods: Between 1991- 2004, 68 patients who underwent SPT with or without PVT, were compared for age, sex, blood counts (Hemoglobin, platelet) and splenic weights. Patients with PVT were evaluated for clinical features, treatment modalities and outcomes. Results: Among 68 SPT, patients with PVT (n: 4, %5.88) and without PVT (n:64) had a mean age and female/male ratio of 13.2 (10-16 years) and 4:0, 10.2 (1-16 years) and 29:35 respectively. Postoperative platelet levels and splenic weights of patients with PVT was 804x103mm3 and 752.5gr and greater than without PVT (465.2x103mm3 and 441gr). Abdominal pain, fever, nausea and vomiting were the initial symptoms (n:3). An asymptomatic patient was diagnosed on routine doppler ultrasonography (DUSG). No patients needed surgery. Diagnosis was made by DUSG (mean 4. day, 2-9 days). Reduced Protein C, S and Antithrombin III and increased Di-Dimer levels was detected in patients with PVT. Antiplatelet agents (acetylsalicylic acid-1.5 mg/kg/day) and antithrombotic therapy (low-molecular-weight heparin- 1 mg/kg/day) were administrated. During follow-up (6 months-6 years) one patient was free of thrombosis, two developed cavernamatous transformation and portal hypertension and one developed partial thrombosis. Conclusion: PVT is a rare but possible complication after SPT. Postoperative thrombocytosis and greater splenic mass are well-known risk factors. According to our results female gender and decreased levels of coagulation inhibitors can be considered as risk factors. DUSG has to be performed in all patients as a diagnostic tool. Patients with PVT must be closely followed-up for cavernous transformation and portal hypertension.
