Abstract

Aim: Surgical correction of esophageal atresia (EA) and tracheoesophageal fistula (TEF) is an integral part of newborn surgery. Many centers perform bronchoscopy routinely to EA patients prior to thoracotomy. Between 2000 and 2005, seventeen of 24 infants with EA+TEF which were treated surgically underwent bronchoscopy and esophagoscopy prior to thoracotomy. Observations and influence of endoscopy on operative techniques and management are discussed. Material and Method: Rigid bronchoscopy and esophagoscopy were performed to seventeen of 24 infants with EA prior to thoracotomy and presence of distal and/or proximal TEF, trifurcation, quadrifurcation, anatomic abnormalities of tracheobroncheal tree, laryngomalacia, tracheomalacia, presence and severity of tracheobronchitis and position of aortic arch were observed. All the patients were evaluated with echocardiography and babygram prior to surgery. Results: Various degrees of tracheobronchitis were observed in 11 of 17, tracheomalacia in 4, ectopic right upper bronchi in 1 infant during bronchoscopy. Three patients had trifurcation of trachea. Distal TEF was detected in 15 patients. No fistula was observed during bronchoscopy and esophagoscopy in two patients and perioperative observation revealed isolated EA during thoracotomy. All but one patient underwent thoracotomy after endoscopy. No abnormality of aortic arch was detected during our study. Esophagoscopy and bronchoscopy were not performed in other 7 patients. No abnormality of aortic arch was detected, radiology revealed tracheobronchitis in two patients of this group. Conclusion: No complications occurred because of bronchoscopy and esophagoscopy. Bronchoscopy changed the plan of operation in one of the 11 patients with tracheo-bronchitis. The patient underwent medical therapy prior to surgery. Bronchoscopy changed the plan of surgery in 5.8% of the patients.
