Abstract

Aim: Hernia of Morgagni is a rare form of congenital diaphragmatic hernia. The incidence is less than 5 % of all diaphragmatic hernias. It is more rarely diagnosed in children. Therefore, the reports on this type of defect are scarce. This report gathers the relevant experience of a pediatric surgical clinic. Material and Method: The operative logs and file records of 8 cases operated on between the years 1996 and 2006 are inevstigated. The presenting complaints, diagnostic investigations, associated anomalies, operative findings and postoperative follow up are recorded. Results: The median age at presentation was 24 months (4 months-8 years). There were 6 (75 %) males and 2 (25 %) females. The presenting complaints were tachypnea, respiratory tract infection and/or vomiting in 7 patients. There was 1 asymptomatic case that was suspected by a plain radiography taken for an unrelevant indication. Associated Down's Syndrome was detected in 3 (38 %) cases. The initial diagnostic investigation was plain thoracic radiographies. These were followed at least by one of upper and/or lower gastrointestinal tract contrast studies and computerized tomography (CT) investigation. Transabdominal operative route was preferred in all cases. The defect was right-sided in 5 (62.5 %) patients, left-sided in 1 (12.5 %) and bilateral in 2 (25 %). Primary repair followed the removal of the sac in all cases. There was no early postoperative complications or mortality. The median follow up period is 7.5 years (9 months-8 years). No late recurrence occured. Conclusion: Hernia of Morgagni usually presents with subacute airway symptoms in children. Down's Syndrome association has a high incidence. Direct radiographies as well as CT scanning are helpful in diagnosis. Even in asymtomatic cases, surgery is indicated after diagnosis. Transabdominal route is especially superior in surgical repair of bilateral cases. Both short- and long-term results of surgery are satisfactory.

3 (% 38) olguda Down Sendromu eşlik etmekteydi. Tüm olgularda öncelikli olarak düz akciğer radyografileri çekilmişti. Bunu takiben, yine tüm olgularda üst veya alt sindirim sistemi kontrastlı görüntülemesi ile bilgisayarlı tomografi (BT) görüntüleme yöntemlerinden en az birisi çalışılmıştı. Tüm olgular trans abdominal yolla ameliyat edildi. DefektS (% 62.5) olguda sağda, 1 (% 12.5) olguda solda ve 2 (%25) olguda çift taraflıydı. Tüm olgularda kese çıkarılarak primer onarım yapıldı. Erken ameliyat sonrası komplikasyon veya mortalite görülmedi. Olguların ortanca izlem süresi 7,5 (9 ay-8 yıl) yıldır. Geç nüks görülmemiştir. Sonuç: Morgagni Hernisi özellikle çocuklarda subakut solunum yolu bulgularıyla seyreder. Down Sendromu birlikteliği sıktır.
