Abstract

Aim: Children with fecal incontinence (FI) due to chronic constipation usually are treated by either pediatric surgery or pediatric psychiatry and via totally different treatment strategies. In the present study we evaluated the role of anorectal physiology and bowel habits of children with FI in determining optimal behavioral and medical treatment approach. Material and Method: 47 patients (11 female, 36 male) who were in the range of 5-14 years with FI admitted to departments of pediatrics and pediatric psychiatry in our center were evaluated by pediatric surgery department for colonic transit time (CTT) and anorectal manometry (AM). The patients were randomly distributed to two groups. Group 1 received behavioral treatment; group 2 received imipramine treatment. Weekly defecation numbers (WDN), fecal and urinary incontinence episodes were recorded pre and post treatment. Results: There were no difference among groups in terms of WDN, CTT, and ARM. FI episodes were reduced in both groups after the treatment but there was no statistical difference between the groups. The children refractory to treatment and with long CTT in group I were found to be younger children. All of the patients in group 2 benefited from imipramine treatment independent from the CTT. Urinary incontinence was found to be significantly lower in patients in group 1. Conclusion: Behavioral treatment in preschool children with fecal incontinence may not be sufficient alone. In these children imipramine may be a suitable adjunctive therapy. Anorectal physiological studies have no statistically significant role in determining the therapy of choice in pediatric patients with FI.
