Abstract

Objective: Direct posterior restorations are still the first choice for tooth disease rehabilitation, thus factors which might affect their clinical performance should be considered to improve longevity. The purpose of this cross-sectional study was to evaluate the effects of factors on clinical performance of direct posterior restorations. Methods: 283 amalgams and 150 composites total 433 direct posterior restorations, which had been performed in different centres, were evaluated in terms of retention, colour match, marginal adaptation, marginal discoloration, anatomical form, secondary caries, postoperative sensitivity and surface roughness using the Modified Ryge Criteria. Variables related to restoration such as material, cavity classification, depth, restoration age, type of clinic centre and variables related to patient such as oral hygiene and smoking were analysed using logistic regression analysis (p=0.05). Results: The failure rates for amalgam and composite were 43% and 39%, respectively. Restorations older than ten years (OR=6.4, p=0.001), Class II (MOD) restorations (OR=9.9, p=0.000) and restorations made in public dental health centres (OR=4.4, p=0.038) were associated with the higher risk for failure in posterior region. Conclusion: Present study showed that amalgam and composite posterior restorations had similar failure rates. Restoration age, cavity classification and type of clinic centre significantly affected the clinical performance of direct posterior restorations.

On yıldan eski restorasyonların (OR = 6.4, p=0.001), Sınıf 2 (MOD) restorasyonların (OR = 9.9, p = 0.000) ve ağız diş sağlığı merkezlerinde uygulanmış restorasyonların (OR = 4.4, p = 0.038) posterior bölgede başarısızlık için yüksek risk ile ilişkili olduğu görülmüştür. Sonuç: Bu çalışma amalgam ve kompozit restorasyonların posterior bölgede benzer başarısızlık oranına sahip olduklarını göstermiştir. Restorasyonun yaşı, kavite sınıflandırması ve uygulandığı merkez direkt yöntemlerle hazırlanan posterior restorasyonların klinik performasını anlamlı olarak etkilemektedir.
