Abstract

Shigella spp. are among the major etiological agents of inf ectious diarrhea, and their inf ections are regarded as a major global public health problem. The aim of this study was to characterize Shigella strains isolated f rom stool samples between 2005-2006 in Ankara, using phenotypic (determining the antimicrobial resistance prof iles) and genotypic (pulsed-f ield gel electrophoresis; PFGE) methods. By employing these results, we aim to acquire a knowledge about the antibiotic sensitivity of the Shigella spp. strains in Ankara that could guide empirical treatment; in addition, we aim to reveal the possible clonal relationship between the isolates by PFGE analysis, to determine the degree of inf ections, and to develop preventive measures against inf ections. A total of 29 Shigella strains (two Shigella dysenteriae and 27 were Shigella sonnei) isolated in 2005 and 2006 were included in the study. The isolates were evaluated f or their sensitivities against 17 diff erent antibiotics. Five diff erent prof iles were determined according to the isolates’ sensitivity. All isolated were sensitive to ciprof loxacin, amikacin, imipenem, tobramycin, gentamicin, chloramphenicol and aztreonama. Five isolates (17.2 %) were resistant to ampicillin, one isolate (3.4 %) was resistant to ampicillin sulbactam, f ive isolates (17.2 %) were resistant to piperacillin, 18 isolates (62 %) were resistant to tetracycline, and 21 isolates (72.4 %) were resistant to trimetoprim-sulf ametoxazole. Four extended spectrum beta-lactamase (ESBL) positive S.sonnei strains were detec- ted among the isolates. PFGE analysis of XbaI-digested genomic DNAs revealed f ive major isolates of S.sonnei and one PFGE prof ile f or S.dysenteriae isolates. According to PFGE f indings; the genotypic homogenity was substantially conserved between the Shigella strains isolated in 2005 and 2006. Considering that various strains were either indistinguishable or closely related, we suspect that the case clusters could share a common bacterial origin, and at least one clone continues to persist. Many isolates were distinct f rom each other in terms of their antimicrobial resis- tance prof iles; yet, their PFGE prof iles were identical or closely-related. This f inding suggests that PFGE provides more accurate results in epidemiological typing compared to antimicrobial resistance prof iles. However, it should be noted that the antimicrobial sensitivity studies using Shigella spp. isolates is always instructive f or the empirical treatment. We believe that there is a need f or additional studies that cover longer terms to evaluate and conf irm the geographical distribution of Shigella spp. strains which cause sporadic cases or epidemics in our country. In addition to the f orementioned studies, determining the clonal relationships between the isolates by genotyping will provide accurate inf or- mation to conf irm epidemics, and detecting the route and the source(s) of inf ection will aid to develop more eff ective methods to control and treat these inf ections.
