Abstract

Introduction: In this study, we aimed to treatiatrogenic pneumothorax via percutaneous drainage catheter insertion with guide wire in percutaneous lung biopsy performed with computerized tomography (CT) and evaluate the efficacy of treatment. Methods: 42 patients with iatrogenic pneumothorax were evaluated retrospectively from 186 patients who under went biopsy. Informed consent was obtained from all patients. The area of the pneumothorax was punctured by an 18G intervention needle from the intercostal space. A 0.035”inch guide wire was advanced through the 18G needle. 8F external drainage catheter was placed over the guide wire and connected to the closed under water drainage system. The patients were followed until the findings of pneumothorax were regressed and their catheters were removed. Results: In 28 patients with pneumothorax, the lesion size was smaller than 2 cm (66,6%) and in 26 patients (62%), the lesions were peripheral and were associated with the development of pneumothorax. In 42 patients who underwent percutaneous drainageca the tertreatment, the technical success of the procedure was 100%. 39 of 42 patients had no complication during the procedure and follow-up. One patient had pneumothorax and pleural bleeding due to biopsy. Minimal asymptomatic pneumothorax developed in 2 patients due to air leakage. Discussion and Conclusion: In tube thoracostomies performed with large diameter chest tubes; malposition, hematoma and bleeding to the pleural space are observed. In the percutaneous catheter the rapies performed with the trocar method, the risk of damage to the intrathoracic organs is high. Guidewire-guided percutaneous drainage catheter treatment is a reliable, effective and minimally invasive method for the management of iatrogenic pneumothorax.
