Abstract

ABSTRACT Aim: Dental anomalies are defined as deviations from the normal development process in the number, size, shape, and structural characteristics of teeth, and may arise as a result of genetic factors, environmental influences, or a combination of these factors. Dental anomalies increase the morphological complexity of the root canal system, making it difficult to effectively perform chemomechanical preparation, irrigation, and obturation procedures. The C-shaped root canal system is a unique anatomical variation characterized by a cross-sectional morphology resembling a C-shape or semicircle, primarily observed in mandibular second molar teeth. The prevalence of the C-shaped root canal system in mandibular second molar teeth ranges from approximately 2.7% to 48.7%. This case series discusses root canal treatments of mandibular second molar teeth with 3 different C-shaped root canal systems. CASE SERIES Case 1: A 39-year-old female patient with no systemic disease. The patient had severe sensitivity in tooth number 37, but no periapical lesion was observed on the radiograph. Examination of the CBCT image revealed a C-shaped root canal system in the relevant tooth. Following shaping, the canal was filled using the thermoplastic obturation method. At the one-year follow-up, the patient had no symptoms. Case 2: The 35-year-old female patient has no systemic disease. Intraoral and radiographic examinations revealed severe pain on percussion in tooth number 37, with a large carious cavity detected in the distal region. A periapical lesion was observed. Following shaping, the canals were filled using the thermoplastic obturation method. At the one-year follow-up, all of the patient's complaints had resolved, and improvement in the lesion was observed compared to the pre-treatment radiograph. Case 3: A 30-year-old male patient with no systemic disease. Radiographic examination revealed deep caries on the mesial surface of tooth number 37 and a periapical lesion at the apex of the tooth. Following preparation, the canal was filled using the thermoplastic obturation technique. At the patient's follow-up appointment one-year later, a reduction in the size of the lesion margins was observed. Conclusion: C-shaped canals are an anatomical variation often seen in mandibular molars. Even though their complex structures it is possible to treat them with non-surgical root canal treatment.
