Abstract

Acinetobacter species are important nosocomial pathogens because they may develop resistance to antibiotics and survive f or long periods in the hospital setting. This study aimed to investigate the changes in antibiotic resistance prof iles of Acinetobacter spp. strains isolated f rom various clinical samples of hospitalized patients in our hospital. The study included a total of 1,498 Acinetobacter species isolated f rom various specimens between June of 2011 and December of 2013. Specimens’ distribution were as f ollows: 540 (36.1 %) bronchial lavage, 290 blood (19.3 %), 174 (11.6 %) wound, 156 tracheal aspirate (10.4 %), 109 throat swab (7.3 %), 81 urine (5.4 %), 52 sputum (3.5 %) and 96 (6.4 %) other specimens (catheter tip, cerebrospinal f luid, thoracentesis material, pleural f luid, peritoneal f luid). Clinical specimens were obtained f rom intensive care units (n=992, 66.2 %) and other clinics (n=141, 30.3 %) of our hospital, there were no outpatient patient specimens. Identif ication and antimicrobial susceptibilities of clinical isolates were determined by an automated system (VITEK 2 Compact, Biomerieux, France) and conventional methods. The isolates were identif ied as Acinetobacter baumannii (n=1,462, 97.6 %), Acinetobacter lwoff ii (n=7, 0.4 %), Acinetobacter haemolyticus (n=2, 0.13 %) and Acinetobacter spp. (n=27, 1.8 %). While resistance rates f or almost all drugs increased, amikacin (67.5 %) and gentamicin (68.6 %) resistance rates decreased and cef tazidime resistance did not change. During all years of the study the most eff ective antibio- tics were colistin (1.4 %) and tigecycline (6.9 %); most ineff ective antibiotics were cef tazidime (93.4 %), piperacillin-tazobactam (93.2%), ciprof loxacin (93.2 %), meropenem (91.6 %), ampicillin-sulbactam (91.4 %), imipenem (91.3 %), cef epime (91.1 %) and levof loxacin (90.9 %), respectively. Lower rates of resistance were observed against amikacin (67.5 %) and gentamicin (68.6 %) in comparison to other antibiotics. This study indicated the presence of high resistance rates against all beta-lactam drugs including carbapenems and quinolones. Moreover, there was a progressive increase in resistance to carbapenems which are very important treatment alternatives. It was concluded that most eff ective antimic- robial therapy may be considered as colistin and tigecycline, while amikacin and gentamicin can also be used f or empirical treatment because of lower resistant rates than other antibiotics.
