Abstract

Objective: In this study, we aimed to evaluate the clinical, laboratory, and radiological features of patients with acute appendicitis who underwent antibiotherapy or surgical treatment and to analyze the efficacy of the nonoperative treatment. Methods: This study included 205 patients who were treated at the pediatric surgery clinic. Age, gender, time from symptom onset to diagnosis, physical examination parameters, white blood cell count, appendiceal diameter, occurence rates of fecaliths, intra-abdominal free fluid, type of appendicitis, surgery, antibiotic treatment, treatment efficacy, recurrence, complications, length of hospital stay and follow-up period were evaluated for all patients. Results: Out of a total of 205 patients, initially 120 (58.5%) patients underwent antibiotic treatment and 85 (41.5%) surgical treatment. The time from symptom onset to diagnosis, findings of diffuse peritoneal irritation during physical examination parameters, frequencies of findings of fecalith and intra-abdominal free fluid, and appendiceal diameter were significantly greater in the group that underwent surgical treatment compared with the group that received antibiotic treatment. Of the 120 patients who underwent antibiotic treatment as the initial treatment, 108 (90%) had successful treatment outcomes. Appendiceal diameter, rates of fecalith formation, white blood cell count and blood glucose levels measured after initiation of treatment were greater in the patients with failed antibiotic treatment. Conclusion: Antibiotic treatment is highly effective for the treatment of patients with uncomplicated acute appendicitis. The treatment should be dynamic and based on the clinical, laboratory, and radiological features of the patient and course of appendicitis rather than standard surgical treatment, and antibiotherapy.

Özet

Amaç: Bu çalışmada, antibiyotik veya cerrahi tedavi uygulanan akut apandisit olgularının klinik, laboratuvar ve radyolojik özelliklerini ve nonoperatif tedavinin etkinliğini değerlendirmeyi amaçladık. Yöntem: Akut apandisit tanısıyla çocuk cerrahisi kliniğinde yatarak tedavi gören 205 olgunun kayıtları değerlendirildi. Olguların tümü, yaş, cinsiyet, karın ağrısının başlamasıyla hastaneye müracaat arasında geçen süre, fizik muayene bulguları, beyaz küre sayısı, apendiksin çapı, fekalit, serbest sıvı, apandisitin tipi, antibiyotik tedavisi, cerrahi tedavi, tedavinin etkinliği, nüks, komplikasyon, hastanede yatış süresi ve takip süresi açısında değerlendirildiler. Bulgular: Toplam 205 olgunun 120’sine (%58,53) başlangıçta yalnızca antibiyotik tedavisi uygulanmış, 85’i ameliyata alınmıştı.
