Abstract

This review aims to synthesise evidence on the risk assessment and clinical applicability of continuous glucose monitoring (CGM) metrics in prediabetes to develop monitoring, risk stratification, and management strategies for preventing progression to type 2 diabetes. The narrative review of CGM studies in prediabetes includes metrics such as data acquisition principles, time in range/above/below, mean glucose, glucose management index, and glycaemic variability indices (standard deviation, coefficient of variation, mean amplitude glycaemic excursions). Clinical challenges, normative data, and the predictive value of dysglycaemia are highlighted. Prediabetes is characterised by increased postprandial excursions and glycaemic variability that can be missed by traditional tests. Compared to normoglycaemia, time spent within the tight range (70-140 mg/dL) is reduced in prediabetes and is associated with insulin resistance. Glycaemic variability indices predict diabetes progression independently of HbA1c. CGM reveals individual glucotypes; practical issues such as sensor wear and data gaps are resolved with extended monitoring. CGM metrics provide comprehensive insights into prediabetic dysglycaemia, surpassing intermittent testing for early diagnosis and intervention. Standardised thresholds and consensus guidelines specific to prediabetes are required for routine clinical use.
