Abstract

Can Oral Antibiotics Be an Alternative to Carbapenems in The Treatment of Non-Complicated Urinary Tract Infections Caused by Extended Spectrum Beta-lactamase Positive Escherichia coli?Increase of extended spectrum beta-lactamase (ESBL) producing Escherichia coli isolates among the causes of urinary system infections and antimicrobial resistance are a major problems. Therefore, difficulties in treatment is observed. Carbapenems are an effective option for treatment of ESBL producing E.coli strains but have features such as being broad spectrum agents, high cost, intravenous/intramuscular injections, and hospitalization requirement. In the treatment of uncomplicated urinary infections, the role of narrow spectrum antibiotics is discussed. In our study; urinary ESBL positive pozitif E.coli strains were tested for the susceptibility against nitrofurantoin, fosfomycin ciprofloxacin and trimethoprim/sulphametoxazole (SMX/TMP). E.coli strains isolated from urine cultures were identified by conventional methods. Antibiotic susceptibilities were determined according to Clinical and Laboratory Standards Institue (CLSI) standards. ESBL producing E.coli strains have been detected by using double disk synergy test. Susceptibility rates were as follows:fosfomycin 96 %, nitrofurantoin 84 %, ciprofloxacin 24 % and SMX/TMP 22 %. Fosfomycin and nitrofurantoin are effective antibiotics for urinary infections. In our study; high sensitivity ratios of fosfomycin and nitrofurantoin were detected. It has been concluded that as an alternative to the use of carbapenems in the treatment of uncomplicated urinary tract infections with ESBL positive E.coli isolates. It may be easy, non-invasive, cost effective and may be beneficial in preventing carbapenem resistance development

Antibiyotik duyarlılıkları Clinical and Laboratory Standarts Institute (CLSI) standartlarına göre belirlenmiştir. İzolatların GSBL üretimi çift disk sinerji testi ile çalışılmıştır. Duyarlılık oranları şu şekildedir: fosfomisin % 96, nitrofurantoin % 84, siprofloksasin % 24 ve SMX/TMP % 22. Üriner infeksiyonlarda özellikle nitrofurantoin ve fosfomisinin etkinliği bilinmektedir. Çalışmamızda da, üriner infeksiyon etkeni GSBL pozitif E.coli izolatlarında fosfomisin ve nitrofurantoine yüksek duyarlılık oranları saptanmıştır. Komplike olmayan üriner sistem infeksiyonlarının tedavisinde karbapenemlere alternatif olarak kullanılmasının kolay, noninvazif, maliyet etkin ve karbapenem direnci gelişiminin önlenmesi açısından yararlı olabileceği sonucuna varılmıştır
