Abstract

Objective: We aimed to calculate the reference change value (RCV) for prostate-specific antigen (PSA) in patients with PSA levels in the gray zone (2–10 ng/mL), compare increases in PSA according to RCV with biopsy results and evaluate the effectiveness of RCV in preventing unnecessary biopsies. Methods: Between January 1, 2018, and December 31, 2019, PSA levels of 148 patients with values in the gray zone were retrospectively analyzed among 1,457 patients whose PSA tests were performed using the Abbott Architect Plus i2000 SR device with the same brand reagent kit. Among these, 56 patients with rising PSA levels were included in the study. The analytical variability (CVa) for total PSA was 9.1%, and according to Westgard, the CV₁ was 18.1%. A Z-score of 1.96 was used for a 95% confidence interval. Based on these data, RCV was calculated by formula RDD:2½.Z.(CVA½+CVI½)½. The RCV was determined to be 56%. A PSA increase of more than 56% between two measurements taken at least three weeks apart was considered biochemically significant. According to the RCV, patients were classified into Group 1 (PSA increase < 56%) and Group 2 (PSA increase ≥ 56%). The relationship between biopsy results and RCV was evaluated using appropriate statistical methods. Results: Among the 56 patients, 52 (92.8%) were in Group 1, and 4 (7.2%) were in Group 2. In Group 1, 37 patients (69.81%) had benign pathology results, while 15 (30.19%) had prostatic adenocarcinoma. In Group 2, 3 out of 4 patients (75%) were diagnosed with cancer. The 56% RCV correctly predicted benign pathology in 69.81% (37/52) of patients, but failed to predict cancer in 30.19% (15/52) of cases. However, in 85% of these 15 cancer cases, the free/total PSA ratio was already below 0.20, supporting malignancy. Ultimately, no statistically significant relationship was found between PSA increases based on RCV and biopsy results (p = 0.093). Conclusion: Although benign pathology results were more common in patients with PSA increases below the RCV, RCV wasn’t a strong enough predictor of cancer before biopsy and shouldn’t be recommended for preventing unnecessary biopsies strongly. Further studies with a larger patient population are needed.
