Abstract

Aim: We aimed to evaluate premature babies who were sent to us for patent ductus arteriosus PDA closure and who were then transferred back following the surgical procedure

Özet

Amaç : Çal›flmam›zda patent duktus arteriyozusun (PDA) kapat›lmas› için sevk edilmifl ve cerrahi sonras› erken dönemde geri gönderilen erken do¤mufl bebekler incelenmifltir. Gereç ve Yöntem: Alt› merkezden gelen 20 erken do¤mufl bebe¤e ameliyathane flartlar›nda PDA kapat›lmas› uyguland›. Olgular geriye dönük olarak incelendi. Olgular›n vücut a¤›rl›¤› 650-1850 (1248,6±398,3) gramd›. Gönderildikleri kliniklere mesafe 5-50 (23,4±20,29) kilometre ve ulafl›m süresi 2,5-28 (12,57±11,44) dakika idi. ‹statistiksel de¤erlendirmede korelasyon analizi, t ve Mann-Whitney U testi kullan›ld›. Bulgular : Cerrahi ve erken dönemde ölüm olmad›. Tüm bebekler 24 saat içinde geri gönderildi. ‹lk iki hafta içerisinde 13 bebek (%65) mekanik ventilasyon deste¤inden ayr›labildi. Üç bebek (%15) sepsis ve kraniyal kanama nedeni ile geç dönemde kaybedildi. Alt› farkl› merkezden klini¤imize gönderilmifl 20 olgunun hiçbirinde hastan›n nakli s›ras›nda hastal›k ve ölüme rastlanmam›flt›r. Operasyon sonras› pnömotoraks nedeni ile gö¤üs tüpü 2. gün nakledildi¤i klinikte çekilen olgu d›fl›nda cerrahi uygulama ile iliflkili hastal›k ve ölüm gözlenmemifl ve tüm bebekler ilk 24 saat içerisinde sevk edildikleri kliniklere geri gönderilebilmifllerdir. Çıkarımlar : Hasta nakil araç ve donan›mlar›n›n geliflimi, sa¤l›k çal›flan›n›n e¤itim ve deneyiminin artmas›na koflut olarak; PDA’n›n cerrahi kapat›lmas› gereklili¤i olan düflük a¤›rl›kl› erken do¤mufl olgularda, hastan›n cerrahi merkeze nakledilmesi ve operasyon sonras› geri gönderilerek tedavisinin sürdürülmesi güvenle uygulanabilir bir seçenektir.
