Abstract

Aim: To evaluate perinatal outcomes in fetuses with lower urinary tract obstruction (LUTO) treated with vesico-amniotic shunting (VAS). Methods: This retrospective case series included singleton pregnancies diagnosed with fetal LUTO that underwent VAS placement between January 2020 and January 2026. Prenatal ultrasonographic findings, amniotic fluid dynamics, fetal therapy details, and pregnancy and neonatal outcomes were obtained from medical records. Due to the limited sample size and clinical heterogeneity, analyses were descriptive. Results: Six fetuses with suspected LUTO underwent VAS placement during the study period. Gestational age at initial diagnosis ranged from 12+0 to 28+1 weeks. Longitudinal bladder diameter at diagnosis varied between 15 and 120 mm, reflecting marked heterogeneity in disease severity. Amniotic fluid volume was within normal limits at diagnosis in all cases; however, oligohydramnios developed during follow-up in one fetus. Procedure-related complications occurred in two cases, including shunt dislocation and shunt blockage. Pregnancy outcomes included two live births, two intrauterine fetal demises, one neonatal death related to extreme prematurity, and one elective termination. Both liveborn infants were delivered at more advanced gestational ages and had preserved amniotic fluid volume throughout pregnancy. Postnatal renal function appeared preserved during early neonatal follow-up in these cases. Conclusion: Perinatal outcomes in fetuses with LUTO treated with VAS remain highly variable. Preservation of amniotic fluid volume and sustained shunt function appear to be important factors associated with improved perinatal survival. Careful patient selection and multidisciplinary prenatal counseling are essential when considering fetal intervention in LUTO.
