Abstract
The treatment process of childhood cancers is complex and requires effective management not only of medical aspects but also of psychosocial and environmental factors. In children under state protection, where family involvement is limited or absent, this process entails additional biopsychosocial and administrative challenges. Clinical experience and literature regarding cancer treatment in this specific patient population remain limited. Therefore, in this report, we discuss the legal regulations in Türkiye and the responsibilities of physicians through the treatment course of a pediatric patient under state protection. A ten-year-old boy living under state protection was admitted with respiratory symptoms and was subsequently diagnosed with T-lymphoblastic lymphoma. No bone marrow or central nervous system involvement was detected. Chemotherapy was initiated according to the COG protocol, and complete remission was achieved at the end of induction therapy. During the treatment process, several challenges not commonly encountered in routine patient care were observed, including the inability to ensure continuity of caregivers, the necessity to conduct informed consent procedures through institutional mechanisms, the need for psychosocial support, and difficulties in coordination between the child care institution and the hospital. Cancer treatment in children under state protection necessitates not only appropriate medical management but also a thorough understanding of the legal framework, close collaboration with social service units, and multidisciplinary teamwork. This case highlights the need for a holistic and patient-centered approach to cancer care in children in need of protection and underscores the importance of increasing awareness of this special patient group within pediatric oncology practice.
