Abstract

Objective: This study aimed to evaluate the frequency of upper gastrointestinal endoscopic findings in patients receiving dialysis treatment due to end-stage renal disease and to compare these findings between hemodialysis and peritoneal dialysis modalities. Materials and Methods: This retrospective study included 141 adult dialysis patients who underwent upper gastrointestinal endoscopy between January 2020 and July 2025. Patients were divided into peritoneal dialysis and hemodialysis groups according to dialysis modality. Demographic characteristics, laboratory parameters, and endoscopic findings were recorded from the hospital database. In addition, patients were compared according to the presence of ulcer. Logistic regression analysis was performed to identify independent factors associated with ulcer presence. Results: Of the patients, 42 (29.8%) were receiving peritoneal dialysis and 99 (70.2%) were receiving hemodialysis treatment. No pathological finding was detected on endoscopic examination in only 7.8% of the patients. Gastritis was the most common endoscopic finding. The frequency of gastritis was similar between the two dialysis modalities. Ulcer was detected in 18.4% of all patients and was significantly more frequent in the hemodialysis group than in the peritoneal dialysis group (23.2% vs. 7.1%; p=0.024). Male sex and serum potassium levels were higher in patients with ulcer. In multivariate analysis, male sex, higher serum potassium level, and hemodialysis treatment were identified as independent predictors of ulcer presence. Conclusion: Upper gastrointestinal endoscopic pathologies are highly prevalent among dialysis patients. Hemodialysis treatment, male sex, and higher potassium levels were independently associated with ulcer risk. These findings suggest that gastrointestinal symptoms should be carefully evaluated and endoscopic examination should not be delayed in high-risk dialysis patients.
