Abstract

Aim: Smallpox and measles have historically had a profound impact on societies due to their high contagion rates, fatal outcomes, and the potential for permanent sequelae. This study aims to examine the diagnostic and therapeutic approaches to smallpox and measles—often discussed together in Ottoman Turkish medical manuscripts from the classical period—within the framework of humoral medicine, and to evaluate these approaches in light of contemporary medical knowledge. Materials and Methods: This study was conducted on Latin-script transcriptions of Ottoman Turkish medical manuscripts, either original compositions or translations, dating from the 14th to the 17th centuries. Within this scope, a total of 51 works were identified through the National Thesis Center of the Council of Higher Education, university libraries, and digital libraries. Data were collected by searching these texts for the keywords “çiçek” (smallpox), “cüderî” (smallpox), “kızamık” (measles), and “hasbe” (measles). The extracted data were then analyzed using content analysis. Findings: In Ottoman medical texts, smallpox and measles are frequently addressed together. Smallpox is described as a sanguineous disease, while measles is associated with a bilious temperament and characterized as an exanthematous condition. The color, shape, and distribution of eruptions were considered crucial indicators of disease prognosis. Black, purple, or hemorrhagic rashes were associated with poor outcomes. Early treatment often involved bloodletting and cupping; if eruptions were delayed, diaphoretic methods were employed. Particular attention was paid to preventing complications. Various ointments were recommended to avoid scarring, and plant- and mineral-based preparations were used to reduce the risk of ocular involvement and blindness. No references to smallpox vaccination (variolation) were found in the examined manuscripts. Conclusion: The symptoms and complications of smallpox and measles described in the analyzed manuscripts largely correspond with modern medical understanding. Especially noteworthy are the plant-, animal-, and mineral-derived remedies proposed for smallpox lesions; investigating their pharmacological effects could reveal their therapeutic potential. Ottoman medicine, with its rich pharmacopeia, should be regarded not merely as a historical legacy but as a dynamic system of knowledge that, through interdisciplinary research, can meaningfully contribute to phytotherapy and traditional and complementary medical practices.
