Abstract

Staphylococcus aureus is a major healthcare problem that can cause infections with high morbidity and mortality rates worldwide. Since, discovery of resistance to methicillin in S.aureus strains, the methicillin resistant S.aureus (MRSA) clones spread to the world and caused serious nosocomial infections. In 90s, different clones of S.aureus that often cause skin and soft tisue infections arose in community. Currently MRSA is an important infectious agent that causes various infections in community as well as healthcare settings. The aim of this study was to compare the antimicrobial resistance profiles of S.aureus strains that were isolated in 2016 and between August 2007 August - July 2008. A total of 376 S.aureus strains were isolated from clinical specimens in Adnan Menderes University Medical Faculty Medical Microbiology Laboratory. A hundred strains were isolated between August 2007 - July 2008 and, 276 strains were isolated in 2016. Fully automated bacterial identification system, Phoenix® (BD Diagnostic Systems, USA), DNase, growth on mannitol salt agar and coagulase tests were used for identification of S.aureus strains. Antimicrobial susceptibilities were determined by the Phoenix® (BD Diagnostic Systems, USA) full automated sensitivity system and disk diffusion method. Susceptibility results were interpreted according to the guidelines of the Clinical Laboratory Standards Institute (CLSI) and The European Committee on Antimicrobial Susceptibility Testing (EUCAST). In our study, we have founded the S.aureus to methicilline resistance 34% between August 2007 - July 2008 and 15.3 % in 2016. In our study, we found that MRSA rates decreased from 34 % to 15.3 % during a time period of nine years (p<0.05). Recently significant reductions of MRSA rates in invasive isolates were reported worldwide, similar to our study. In our study both invasive and non-invasive isolates were included but healthcare assosiated MRSA (HA-MRSA) and community associated MRSA (CA-MRSA) strains could not be differentiated. The reduction of invasive MRSA infections could be linked to improvements in infection control procedures. But it can be insufficient to detect MRSA rates by using data from only invasive isolates. CA-MRSA strains that cause non-invasive conditions, like skin and soft tissue infections are of concern. Therefore there is a need for a large scale epidemiological studies that involve both invasive and non-invasive isolates in order to fully understand if the measures on S.aureus are effective.

Duyarlılık sonuçları Clinical Laboratory Standards Institute (CLSI) ve The European Committee on Antimicrobial Susceptibility Testing (EUCAST) kılavuzlarına göre yorumlanmıştır.
