Abstract

Urinary tract infections can be of community and hospital origin. Klebsiella pneumoniae is among the most common bacteria causing urinary tract infections. Its increasing resistance to antimicrobials is a concern. The aim of this study was to evaluate the antimicrobial resistance patterns of hospital and community-acquired K. pneumoniae strains isolated from urine cultures. The antibiotic susceptibilities of samples with ≥ 104 cfu/ml K. pneumoniae growth in urine cultures sent to the microbiology laboratory between 23.05.2021-23.05.2024 were evaluated retrospectively. Bacterial identification was performed by BD Phoenix 100 bacterial identification automated system and matrix assisted laser desorption ionisation time of flight mass spectrometry (MALDI-TOF MS). Antibiotic susceptibilities were determined by Kirby Bauer disc diffusion method and evaluated according to the recommendations of ‘The European Committee on Antimicrobial Susceptibility Testing’ (EUCAST). The study included 1991 K. pneumoniae isolates. The lowest antimicrobial resistance was observed against amikacin (12.4%), imipenem (16%), meropenem (16.8%); and the highest antimicrobial resistance was observed against piperacillin (74.2%), ampicillin/sulbactam (67.8%), cefazolin (64.7%). When antimicrobial resistance rates were compared according to age groups, resistance rates were higher in patients aged 46 years and older (p˂0.05). Resistance rates to all antimicrobials except aztreonam were higher in hospital-acquired samples (p˂0.05). Resistance to all antimicrobials except piperacillin and cefepime was high in isolates from intensive care unit (p˂0.05). Multidrug-resistance (MDR) was detected in 667 (33.5%) and extensively drug-resistance (XDR) was detected in 485 (24.4%) of the isolates. MDR and XDR rates were higher in intensive care isolates compared to inpatient ward and outpatient clinic isolates (p=0.0001). The rates of MDR and XDR were statistically significantly higher in hospital-acquired isolates compared to community-acquired isolates (p=0.0001). In conclusion, the limited treatment options in the 0-45 age range and the lack of empirical treatment options with advancing age (46 years and older) are worrying. Another worrying situation is the higher MDR and XDR rates of intensive care isolates and hospital-acquired isolates.

Antibiyotik duyarlılıkları Kirby Bauer disk difüzyon yöntemiyle saptandı ve “The European Committee on Antimicrobial Susceptibility Testing” (EUCAST) önerilerine uyularak değerlendirildi.
