Abstract

Objective: This study aimed to evaluate the cerebellar vermis volume in individuals diagnosed with schizophrenia and bipolar disorder and to compare these values with those of healthy controls. Given the cerebellum’s—particularly the vermis’s—critical role in cognitive and emotional processing, examining potential morphological alterations in this region is of considerable significance for neuropsychiatric research. Materials and Methods: This comparative cross-sectional study retrospectively analyzed data from 26 individuals with schizophrenia, 26 with bipolar disorder, and 26 healthy controls who presented to the Psychiatry Outpatient Clinic of Balıkesir University Faculty of Medicine Hospital between 2017 and 2024. Cranial magnetic resonance imaging (MRI) scans were acquired using a Philips Ingenia 1.5 Tesla scanner. T1-weighted sagittal images were processed with the VolBrain software to calculate total cerebellar vermis volumes. Statistical analyses were conducted using IBM SPSS Statistics version 25. Group comparisons were performed using one-way ANOVA, while the Kruskal–Wallis and Pearson correlation tests were applied according to the distributional properties of the data. Results: There was no statistically significant difference in total cerebellar vermis volumes among the schizophrenia, bipolar disorder, and control groups (p > 0.05). No significant correlation was found between age and vermis volume (p = 0.64), whereas a moderate positive correlation was observed between gender and vermis volume (p < 0.05), indicating that males exhibited larger vermis volumes compared to females. Conclusion: The findings suggest that cerebellar vermis volume is structurally preserved in individuals with schizophrenia and bipolar disorder relative to healthy controls. While age did not exert a significant influence, gender appeared to have a meaningful effect on vermis volume. These results indicate that cerebellar involvement in psychiatric disorders may extend beyond volumetric alterations and should also be examined at functional and connectivity levels. Future studies utilizing higher-field MRI systems are expected to yield deeper insights into the cerebellum’s role in the pathophysiology of psychiatric disorders.
