Preoperatif Kırmızı Küre Dağılım Genişliği ile Mide Kanseri Ameliyat Sonrası Erken Dönem Ölüm İlişkisi

Özet
AMAÇ: Çalıșmamızda mide kanserli hastalarda ameliyat öncesi eritrosit dağılım genișliği ölçümünün, postoperatif ilk 30 gündeki erken dönemde mortaliteyi öngörüp göremeyeceğini araștırmayı amaçladık. YÖNTEM: Ocak 2013 ve Kasım 2014 arasında ameliyat edilen mide kanseri olan hastaların kayıtları retrospektif olarak incelendi. 137 hastadan 116 tanesi çalıșma için uygun görüldü ve iki gruba ayrıldılar: Grup 1 ameliyat sonrası ilk ay içinde ölenleri içerirken, Grup 2 sağ kalan hastaları içeriyordu. Tanımsal değișkenlere ek olarak, tümör özellikleri ve kırmızı küre dağı- lım genișliği de gruplar arasında karșılaștırıldı. P değerinin anlamlılığı için <0.05 değeri kullanıldı. BULGULAR: Hastaların ortanca yașı 64’dü. Erkek/kadın oranı 1,9’du. Tümörler çoğunlukla midenin son üçte birlik bölümündeydi ve çoğunlukla adenokanserdi. Yüz on altı hastadan 21’i (Grup 1) ilk 30 gün içinde öldüler. Ortalama kırmızı küre dağılım genișliği değeri Grup 1’de anlamlı olarak Grup 2’den daha yüksekti (p=0.002). ROC eğrisi analizi; kırmızı küre da- ğılım genișliği için 14,75 kesim noktası değerinin mide kanseri için ameliyat sonrası ölümü öngörebilmede sırasıyla %66,7 ve %67,4 duyarlılık ve özgüllüğe sahip olduğunu gösterdi. SONUÇ: Ameliyat öncesi kırmızı küre dağılım genișliğinin artması, mide kanseri hastalarında ameliyat sonrası ölümü öngörebiliyor gibi görülmektedir.
Abstract
AIM: In our study we aimed to investigate whether the preoperative red cell distribution width measurement can predict the early postoperative mortality within the fi rst 30 days in patients with gastric cancer. METHODS: The records of gastric cancer patients operated between January 2013 and November 2014 were retrospectively analyzed. Of the 137 operations, 116 were eligible for the study and allocated into two groups; Group 1 included the patients died within the fi rst month after operation and Group 2 included the survived patients. Besides the demographics, tumor characteristics and red cell distribution width were compared between groups. P<0.05 was set as the signifi cance level. RESULTS: The median age of the patients was 64. Male/female ratio was 1.9. The tumors were commonly localized at the distal one third of the stomach and most of them were adenocarcinoma. Of the 116 patients, 21 died (Group 1) within the fi rst 30 days of postoperative period. Mean red cell distribution width value in Group 1 was signifi cantly higher than in Group 2 (p:0.002). ROC curve analysis revealed that the cut-off value of 14.75 for red cell distribution width has a sensitivity and specifi city of 66.7% and 67.4%, respectively, to predict the early postoperative mortality in gastric cancer patients. CONCLUSION: It seems that the rise in preoperative red cell distribution width can predict the early postoperative mortality in patients diagnosed with gastric cancer.
