Organofosfat Zehirlenmesinde Yeni Bir Tedavi Yaklaşımı: Fitoterapi

Özet
Dünya Sağlık Örgütünün raporlarında, her yıl yaklaşık 3 milyon insanda pestisit zehirlenmesi meydana geldiği ve bu zehirlenmelerin 220.000’inin ölümle sonuçlandığı belirtilmektedir.Ülkemizin genellikle kırsal kesimlerinde en sık rastlanan pestisit zehirlenmesi organofosfat zehirlenmesidir. Organofosfatlar memelilerle karşılaştırıldığında böceklere daha seçici toksisite göstermeleri ve sınırlı düzeyde kalıntı oluşturma özelliklerinden dolayı ülkemizde ve dünya genelinde yaygın şekilde kullanılmaktadır. Organofosfatlar dokularda asetilkolinesteraz enziminin (AChE) inhibisyonuna neden olarak asetilkolin birikimine yol açar ve böylece nörotoksik bir etki meydana getirir. Ayrıca organofosfatların akut solunum yetmezliği, hepatotoksisite, genetik toksisite, teratojenite, immunotoksisite, pankreatit, hipoglisemi, konvülsyon ve orşit gibi farklı sistemler üzerinde olumsuz etkilere neden olduğu bildirilmiştir. Organofosfat zehirlenmelerinde klasik tanı ve tedavi yöntemleri bilinmektedir. Ancak fazla sayıdaki bu olumsuz etkiye karşı tanı ve tedaviye destek sağlayacak ve prognozu olumlu yönde etkileyecek yeni metod ve ilaç çalışmalarına ihtiyaç duyulmaktadır. Bu amaçla makalemizde; organofosfat zehirlenmesinin genel özellikleri, klinik tanı-tedavi yöntemleri ve ayrıca rutin tedaviyi destekleyebilecek, ilaç çalışmalarına yön verebilecek yeni ve alternatif koruyucu/tedavi edici bir yöntem olan "bitkisel tedavi" yönteminden bahsedilmektedir. Yapılmış olan rat çalışmaları ile organofosfat zehirlenmesine karşı etkileri bilinen ve antioksidan özellikleri olan çeşitli bitkisel ilaçların, insanlarda da uygulanabilir hale gelmesi için yeni çalışmalar yapılmalıdır. Antioksidan özelliği olan bu bitkilerin organofosfat toksisitesi üzerine etkilerinin araştırılması ve ortaya çıkan bulguların yorumlanarak insanlarda uygulanabilecek yeni tedavi yöntemlerinin geliştirilmesi gerekmektedir.
Abstract
According to the World Health Organization reports, the pesticide poisoning occurs every year in approximately three million people and it is stated that 220 000 poisonings of them resulted in death. Especially in rural areas of our country, the most common pesticide poisoning is organophosphate's. It is widely used in our country and around the world due to the properties that leaves remnant in limited amount and to creating a more selective toxicity against insects compared to mammals. Organophosphates leads to the accumulation of acetylcholine as a result of the enzyme acetylcholinesterase (AChE) inhibition in tissues, and thus produces neurotoxic effects. In addition, organophosphates have been reported to cause adverse effects on the different systems such as acute respiratory failure, hepatotoxicity, genetic toxicity, teratogenicity, immuno toxicity, pancreatitis, hypoglycemia, seizures and orchitis. Only the classic diagnosis and treatment modalities is known at organophosphate poisoning. However, in spite of a greater number of its negative effect, new methods and pharmaceutical studies are needed those will support the diagnosis and treatment, and impose a positive influence on prognosis. For this purpose, in the article, it is also mentioned a new and alternative pharmaceutical preventive / therapeutic method like "herbal treatment" to support the routine treatment in addition to general characteristics of organophosphate poisoning. New studies should also be conducted in humans to become applicable of the herbal medicines which have the effects of a variety of known and antioxidant properties against organophosphate poisoning. The resulting findings, gained by investigating the effects of these plants with antioxidant properties on the organophosphate toxicity, should be interpreted with the need to develop new treatment methods that canbe applied in humans.
