Özet
Methemoglobinemi yenidoğan döneminde siyanoza neden olabilen, ender bir hematolojik hastalıktır. Konjenital ya da edinsel olmak üzere iki tipi bulunan methemoglobinemi yenidoğan döneminde en sık ilaç reaksiyonu olarak ortaya çıkmaktadır. Literatürde, lokal anestezik olarak yaygın biçimde kullanılan prilokainin, yenidoğan bebeklerde edinsel methemoglobinemiye yol açabildiği bildirilmektedir. Sünnet öncesi lokal anestezik olarak prilokain uygulanan ve siyanoz gelişen 10 saatlik bir yenidoğan bebek, methemoglobinemi tanısı konularak intravenöz askorbik asit uygulaması ile başarıyla tedavi edilmiştir. Ender görülen bir klinik durum olan edinsel methemoglobinemi ve tedavisi literatür eşliğinde tartışılmıştır.
Abstract
Methemoglobinemia is a rare hematological disease that can cause cyanosis in the neonatal period. The disease has two different types, congenital and acquired, and in the neonatal period it most frequently occurs because of the drug reactions.In the literature, it has been reported that prilocaine, a frequently used local anesthetic, may lead to acquired methemoglobinemia in newborn infants. A 10-hour old newborn infant, who developed cyanosis after the prilocaine injection to induce local anesthesia before his circumcision procedure, had the diagnosis of methemoglobinemia and treated successfully with intravenous ascorbic acid administration. Acquired methemoglobinemia, which is a rare clinical entity, and its treatment are discussed in the light of the literature.
