Özet

Amaç: Amacımız intravenöz (iv) deksametazon kullanarak kaudal blok süresine etkisini araştırmaktır. Yöntemler: Bu prospektif, randomize, çift-kör çalışma; elektif infraumbilikal cerrahi geçirecek 1-9 yaş arası 40 hasta üzerinde gerçekleştirildi. Hastalar Grup D ve Grup K olarak randomize edildi. Entübasyon sonrası Grup D’ye (n=20) 0,15 mg/kg iv deksametazon, Grup K’ya (n=20) aynı volümde iv normal salin verildi. Tüm hastalara %0,25 bupivakain, 1 ml kg-1 volümde hazırlanarak kaudal anestezi yapıldı. Cerrahi sonrası 48 saat ağrı skorları, postoperatif analjezik ihtiyaçları değerlendirildi. Bulgular: Postoperatif FLACC ağrı skorları (0, 1, 2, 4, 8, 12, 24 ve 48. saatler) her iki grupta benzerdi (p>0,05). Cerrahi sonrası ilk 24 saatte Grup K’da 6, Grup D’de ise 3 hastada analjezi ihtiyacı oldu (p>0,05). İkinci 24 saatte ise hiçbir hastada analjezik ihtiyacı olmadı. Sonuç: Deksametazon uygulamasının ek bir katkısı görülmemiştir.

Abstract

Aim: We aimed to research the effect of intravenous(IV) dexamethasone on duration of analgesia provided by caudal block. Methods: This study was designed as prospective, randomized and double blinded format and carried out on fourth patients with 1 – 9 years of ages. The patients randomized into two groups, as Group D and Group K. After intubation, Group D (n=20) received 0.15 mg/kg IV dexamethasone and Group C received same volume of normal saline. Caudal analgesia was achieved with 1 ml kg-1 0.25% bupivacaine. In the postoperative period of 48 hours, level of analgesia and need for analgesic drugs were evaluated. Results: Postoperative FLACC tool values were similar in both groups (0th, 1th, 2th, 4th, 8th, 12th, 24th and 48th. hours) (p>0.05). After the first 24 hours period of the surgery, there were a need for analgesia in six patients of Group K and three patients of Group D. There were no needs for analgesia for the second 24hours. Results: Addition of dexamethasone does not contribute much in these patients
