Dirençli Hiperkalsemi İle Presente Olan Bir Multiple Miyelom Vakası

Özet

48 yaşındaki kadın hastan ın hipertansiyon öyküsü dışında bir hastalığı bulunmamaktadır. Özgeçmişinde 2 haftalık hiperkalsemi öyküsü vardı. Bu süre zarf ında ba şka bir merkezde çal ışan bir iç hastal ıkları uzmanı, hiperkalsemi ve akut böbrek hasarı (ABH) ile hastayı iki kez birer hafta arayla yatırmış. En son ata ğında 3 günlük yatıştan sonra böbrek fonksiyon testleri normale gelmiş. Hiperkalsemi ısrar edince, taburcu edilip hastanemize sevk edilmiş. Halsizlik, sırt-bel ağrısı, yürüme güçlüğü, şiddetli hiperkalsemi (17.7 mg/dL) ve ABH ile acilimize getirildi. Fizik muayenesinde arteriyal tansiyon yüksekliği ve kemik ağrıları bulundu. Nefroloji servisimize yat ırıldı. Hidrasyon ile böbreğin fonksiyon testleri düzeldi. Ancak, hiperkalsemi hidrasyona ve furosemide yanıt vermedi. Bifosfonat, kortikosteroid, kalsitonin gibi anti-hiperkalsemik tedavi protokolleri denenmesine rağmen hiperkalsemisi düzene girmedi. Bu tedaviler kesilince serum kalsiyum düzeyi tekrar yükseliyordu. Dirençli hiperkalsemi düşünüldü. Hiperkalsemi etyolojik analizinde, kemik iliği ince i ğne aspirasyon tetkikiyle multiple miyelom (MM) tanısını aldı. Siklofosfamid, bortezomib ve deksametazon kemoterapi kürü verildi. Sonras ında serum kalsiyum düzeyi düzene girebildi. MM çoğunlukla ileri yaş erkeklerde görülür. Vakamızın genç bir kadın olması ve dirençli hiperklasemisi onu e şsiz kılmaktadır.

Abstract

48-year-old female patient has no disease other than a history of hypertension. He had a history of hypercalcemia for 2 weeks. During this period, an internist working in another center hospitalized the patient with hypercalcemia and acute kidney injury (AKI) twice, with a week 's interval. Renal function tests returned to normal after 3 days of hospitalization in the last attack. When hypercalcemia persisted, she was discharged and referred to our hospital. She was brought to our emergency with weakness, back pain, walking difficulty, severe hypercalcemia (17.7 mg/dL) and AKI. On physical examination, high arterial blood pressure and bone pain were found. Hospitalized to our nephrology service. Fu nction tests of the kidney improved with hydration. However, hypercalcemia did not respond to hydration and furosemide. Although anti-hypercalcemic treatment protocols such as bisphosphonate, corticosteroid, and calcitonin were tried, hyperc alcemia did not regulate. When these treatments were discontinued, the serum calcium level rose again. Resistant hypercalcemia was considered. In the etiological analysis of hypercalcemia, multiple myeloma (MM) was diagnosed with bone marrow fine needle aspiration examination. Cyclophosphamide, bortezomib and dexamethasone chemotherapy cure was given. Afterwards, the serum calcium level could be regulated. MM is mostly seen in older men. The fact that our case was a young woman and her resistant hypercalcemia make its unique.
