Kronik miyelositer lösemi hastalarında sitogenetik analiz ve P53 kodon 249 mutasyon analiz sonuçları

Özet
Kronik miyelositer lösemiler (KML) miyeloid hiperp-lazi, granülositer serinin yapımında artış ve sitoge-netik olarak "Philadelphia" Ph1 kromozomunun [t (9;22)(q34;q11)] gösterilmesi ile karakterize bir kök hücre hastalığıdır. 9. ve 22. kromozomlar üzerinde bulunan bcr ve abl proto-onkogenleri resiprokal bir translokasyon sonucu 22. kromozom üzerinde bir füzyon geni oluşturmakta ve ortaya çıkan protein (p210) hücrede malign transformasyonu başlatmak¬tadır. Olgular hızlanmış faz ve ardından gelişen blastik kriz sonucunda kaybedilmektedir. Son yıllarda kronik miyelositer lösemi olgularında blastik kriz aşamasında p53 genine ait mutasyonlar bildirilmek¬tedir. Bu çalışmada ilk tanı ve tedavi sonrası dönemde periferik kan ve kemik iliği örnekleri alınan 5 kronik miyelositerlösemi olgusunda kromozom analizi ve p53 mutasyon analizleri yürütülmüştür. Bu 5 olguda ilk tanı evresinde Ph1 kromozomu saptan-mıştır.PM kromozomu pozitif olan 4 olgu tedavi ile remisyona girmiş, remisyon evresi kromozom çalış¬masında Ph1 kromozomu negatif olarak saptan¬mıştır. Hızlanmış faza giren 1 olguda ise Ph1 kromozomunun devamlılığı gösterilmiştir. Olgularda Ph1 kromozomu haricinde saptanan klonal bir sitoge-netik anomali yoktur. p53 ekson 7 nin SSCP çalışma¬sında 5 olguda Hea III restriksiyon enzimi ile yapılan analizde ilk tanı evresinde normal pattern tespit edil¬miştir. Remisyona giren 4 olguda yine p53 ekson 7 kodon 249 mutasyohu tespit edilmemiş, hızlanmış faz evresindeki tek olguda ise bu mutasyon tanımlan¬mıştır. Bulgular kronik miyelositer lösemi olgularında hızlanmış faz ve p53 mutasyonları arasındaki ilişkiyi doğrulayıcı niteliktedir.
Abstract
Chronic myeloid leukemia is characterized by myeloid hyperplasia, expansion of the total granu-locyte pool and existence of a specific chromosome translocation [t(9;22)(q34;q11)], Philadelphia Chro¬mosome (Ph1). Chronic myeloid leukemia starts with a reciprocal translocation between chromosome 9 and 22; c-abl and the protein product (p210) causes the proliferation and malignant transformation of the cells. Patients usually die after a blastic crisis follow¬ing an accelerated phase. There is a positive correla¬tion between p53 mutations and blastic crises in chronic myeloid leukemia. In this study, five chronic myeloid leukemia patients' peripheral blood and bone marrow samples were studied cytogenetically at initial diagnosis phase and after treatment. All five patients had Ph1 chromosome in initial diagnosis phase. Four of the Ph1 chromosome positive pati¬ents showed remission with therapy and became Ph1 chromosome negative, but one went into the accelerated phase and the Ph1 chromosome persisted. No other clonal cytogenetic aberration except Ph1, was found. p53 exon 7 SSCP analyses of five patients revealed normal pattern in both the initial diagnosis phase and after the treatment. According to the analyses with Hea III restriction enzyme cleavage, four patients who gained remis¬sion showed no p53 exon 7 codon 249 mutation before and after the treatment. Hea III enzyme clea¬vage showed p53 codon 249 mutation only in the patient who moved into accelerated phase in spite of the treatment. Exon 7 codon 249 of this patient was normal during the initial diagnosis phase. Correlation between the development of the accelerated phase, patient's clinical stage and the evolution of p53 muta¬tion is stressed.
