Akut immun trombositopenik purpurada tek doz intravenöz immunglobulin tedavisinin etkinliği

Özet

Akut immun trombositopenik purpura (İTP) çocukluk çağının en sık rastlanan edinsel kanama bozukluklarından birisidir. Akut İTP'nin en korkulan komplikasyonu intrakranial kanama gelişimidir ve bu risk- özellikle trombosit sayısı 20.000/mm3 ün altında olan çocuklarda yüksektir. Bu nedenle akut İTP tedavisinde en önemli amaç trombosit sayısını en kısa sürede 20.000/mm3 ün üzerine çıkarmaktır. Bu çalışmada kısa sürede trombosit sayısını yükseltebilmek ve maliyeti azaltabilmek amacına yönelik olarak tek dozda intravenöz immunglobulin tedavisinin etkinliği prospektif olarak değerlendirildi. Trombosit sayısı 20.000/mm3 ün altında olan veya 20.000/mm3 ün üstü ve uzamış kanama zamanı veya aktif kanaması olan hastalara tedavi verildi. Akut ITP tanısı alan ve yaş ortalaması 5.0±3.74 yıl olan 47 çocuğa (25 erkek, 22 kız) İVİG 0.8 gr/kg/gün dozunda (süt çocuklarına 0.4 gr/kg/gün) uygulandı. Tedaviden 24 saat trombosit sayısı 20.000/mm3 üstünde, kanama zamanı normal ve aktif kanaması olmayan hastalarda tedavi başarılı olarak değerlendirildi. Bu kriterler 13 hastada elde edilebildi (Grup 1). ikinci doz İVİG tedavisi 19 hastada gerekirken (Grup 2), 6 hastaya üçüncü doz da verildi (Grup 3). Son grupta bir hastada üç doz İVİG tedavisine cevapsızlık nedeniyle steroid tedavisine geçildi. Dokuz hasta ise süt çocuğu idi (Grup 4) ve beşine 2 gün, dördüne 3 gün İVİG verildi. Hastaların takibinde 3 vaka kontrollerine gelmedi, 4 vaka ise sevk alamadıklarından başka hastanelerden takipleri için sevk edildi. Diğer vakaların takibinde ise 29/40 (%72.5) vakada bir daha tedavi gereksinimi olmadı. Yedi hasta (7/40, %17.5) kronik İTP tanısı aldı. Hiçbir hastada hayatı tehdit eden kanama gözlenmedi. Sonuç olarak; tek doz İVİG tedavisi ile istenen yanıt (infant grubu dışında) 13/38 (%34.2) vakada sağlanabilirken, ikinci doz ilavesi ile istenen yanıt 32/38 (%84.2) hastada elde edilmiştir.

Abstract

Acute immune thrombocytopenic purpura is one of the most common acquired bleeding disorders in childhood. One of the most feared complications of ITP is development of intracranial hemorrhage (ICH). This risk is high in children whose platelet counts are less than 20.000/mm3. In this study, the effectiveness of a single dose of IVIG was assessed prospectively to obtain a rapid increase in platelet count and to decrease the cost of therapy. Our criteria for starting the therapy was as follows: Platelet count 20.000/mm3 and prolonged bleeding time or the patients with active bleeding. IVIG was administered in a single dose of 0.8 g/kg/day (0.4 g/kg/day for infants) to 47 children (25 male, 22 female) who were diagnosed as acute ITP. The mean age of the patients was 5.0±3.74 years. After 24 hours the success or the cessation criteria of the therapy was defined as the platelet count >20.000/mm3 with normal bleeding time and no active bleeding. A single dose was enough to achieve success in 13 patients (Group 1). A second dose of 0.8 g/kg/day was administered to 19 patients (Group 2) due to persistence of platelet count 20.000/mm3 and prolonged bleeding time or persistence of active bleeding. A third dose was applied to 6 patients (Group 3), and failure was obtained in one patient in this group, and corticosteroid therapy was added to this patient, then he responded. For infants (n: 9); IVIG was administered two days for 5 patients, and three days for 4 infants (Group 4). Three patients were lost in follow-up, and four patients were sent to the another hospitals due to their health insurance. At the follow-up of other patients, steady platelet count could be achieved in 29/40 (72.5%) patients. Chronic ITP was diagnosed in 7/40 (17.5%) patients. We did not observe any life-threatening bleeding. In conclusion, a single dose of IVIG only was enough in 13/38 (34.2%) patients except Group 4, and after the addition of second dose of IVIG, success was obtained in 32/38 (84.2%) patients.
