Akut İliofemoral Derin Ven Trombozunun Farmakomekanik Trombektomi Yöntemiyle Endovenöz Tedavisinde Erken Dönem Sonuçlarımız: Tek Merkez Deneyimi

Özet
ÖZET Amaç : Derin ven trombozu (DVT) hayatı tehdit eden komplikasyonları olan ve hastanede de en çok önlenebilir mortalite nedenlerinden biridir. Bu çalışmada akut iliofemoral DVT olan hasta- larda farmakomekanik trombektomi uygulamalarımızın erken dönem sonuçlarını sunmayı amaçladık. G ereç ve Yönt emler: Kliniğimizde Aralık 2012-Kasım 2014 tarihleri arasında endovenöz yolla re- kombinant insan doku plazminojen aktivatörü eşliğinde rotasyonel trombektomi katateri ile farma- komekanik trombektomi uyguladığımız 30 hasta (16 erkek, 14 kadın; ortalama yaş 54,4±14,8 yıl, aralık 29-82 yıl) retrospektif olarak incelenerek çalışmaya dahil edildi. Çalışmaya sadece ilk 15 gün içindeki akut DVT hastaları dahil edildi. Tüm hastalara pulmoner emboli komplikasyonunu önlemek için ge- çici vena cava filtresi takıldı. İşlem sonrası hastalara oral warfarin tedavisi başlandı, ve INR seviyesi 2-3 arasında olan hastalar taburcu edildi. Bulgu lar : İşlem sonrası girişim yerinde hematom olan 2 hasta dışında majör kompliklasyon yada mortalite izlenmedi. Hastalar 1, 3, 6. aylarda ve daha sonra yılda bir kez olmak üzere Doppler ultrasonografi ile takibe alındı. 1.yıl sonunda hastaların %97sinde tam açık- lık sağlandığı gözlendi. So nuç: Akut DVT tedavisinde ilk tercih antikoagülan tedavidir, fakat DVTde verilen antikoagülan tedavinin trombolitik etkisi yoktur ve rekanalizasyon sadece endojen fibrinoli- tik sistemin etkinliğine bağlıdır. Özellikle akut trombozda; trombüs kütlesinin tama yakın yok edile- bilmesi, tromboz komplikasyon ihtimalinin azaltılması, hastanede kalış süresinin kısalması yönünden farmakomekanik trombektomi yöntemlerinin avantajlı olduğunu düşünmekteyiz. Abstract
ABSTRACT O b jective: Deep vein thrombosis has life-threatening complications, and it is one of the most common preventable causes of in-hospital mortality. In this study, we aimed to present short term results of pharmacomechanic thrombectomy procedures in patients with acute ileofemoral deep vein thrombosis (DVT). Mater ial and Methods: Thirty patients (16 males, 14 females; mean age 54.4±14.8 years, range. 29-82 years) who had pharmacomechanic thrombectomy with rotational thrombectomy catheter together with endovenous recombinant human tissue plasminogen activator between December 2012-November 2014 were analyzed retrospectively, and included in the study. Acute DVT patients were included in the study only in the first 15 days of the disease. A temporary vena cava filter was placed in all patients to prevent pulmonary embolism. Patients were adminis- tered warfarin after the operation, and the ones with an INR level of 2-3 were discharged. Results: No major complications or mortality were observed except 2 patients who developed hematoma at in- tervention site after the procedure. Patients were followed up with Doppler ultrasonography at post- operative 1st, 3rd, 6th months, and then once a year. At the end of 1st year, the patency rate was 97%. Conclusion: Anticoagulants are the first line treatment for acute DVT, however anticoagulant treat- ment does not have a thrombolytic effect and recanalization depends only on the efficiency of en- dovenous thrombolytic system. We think that pharmacomechanic thrombectomy methods are advantageous for almost complete disappearance of thrombus in particularly acute thrombosis, they reduce the risk of complications related to thrombosis, and shorten hospital stay.
