Periferik arter pseudoanevrizmalarının etiyolojik sınıflaması, klinik ve cerrahi yaklaşım: 104 olguluk klinik deneyimimiz

Özet
Amaç: Periferik arterlerin pseudoanevrizmalarında etiyolojik sınıflama, risk faktörleri ve tedavi yaklaşımlarının kendi klinik tecrübelerimiz doğrultusunda literatür eşliğinde değerlendirilmesi. Yöntem: Ocak 2000 - Şubat 2005 tarihleri arasında kliniğimizde opere edilen 104 pseudoanevrizma olgusu etiyolojilerine göre sınıflanarak tedavi yaklaşımları gözden geçirildi. En sık gözlenilen yerleşim yeri common ve superficial femoral arterdi (56 olgu %53.8). Yirmidokuz olguda kesici-delici alet yada ateşli silah yaralanması (%27.9), 8 olguda trafik kazası (%7.7), 1 olguda duvar göçüğü altında kalma (%0.9), 6 olguda greft anastomoz hattından (%5.8), 19 olguda angiografi sonrası (%18.3) ve 41 olguda KBY nedeni ile açılan fistül yeri (%39.4) pseudoanevrizma nedeni olarak tespit edildi. Temelonarım planı pseudoanevrizma gelişen segmentin çıkarılarak kalan kısımlar uc uca yada araya safen greft interpozisyonu ile devamlılığın sağlanması idi. Uygulanamayan olgularda sentetik greft interpozisyonu yapıldı. Bulgular: Sekiz olguda primer etiyoloji içerisinde yer alan enfeksiyona bağlı sekonder yara yeri enfeksiyonları gelişti. Altı olguda kültür (+) olarak saptandı. İki olgu enfeksiyon nedeni ile debridman amacı ile yeniden opere edildi. Dört olguda ise geniş doku defekti nedeni ile doku fleblemesi yapıldı. Operasyon sonrası damarların açık kalımı ve distal dolaşım klinik muayeneye ek olarak Doppler USG ile değerlendirildi. İki olguda rekürren pseudoanevrizma gelişti. Sonuç: Travmatik yada iatrojenik etiyolojisi ne olursa olsun pseudoanevrizmanın erken tanısı ve acil cerrahi tedavisi çok önemlidir. Tanı ve tedavi süresi uzadıkça hem cerrahi girişim daha komplike hale gelmekte hem de doku ve/veya yaşam kaybı riski artmaktadır.
Abstract
Purpose: The aim of the study is to evoluation of ethiological classification, risk factors and the treatment options of the pseudoaneursyms of the peripheric arteries within litheratur and our expenences. Methods: We operated 104 patients for pseudoaneurysm ofperipheric arteries between January 2000 and February 2005. Common and superficial femoral arteries are the most often settlement artery (56patients 53.8%). The ethiological classification of our patients are; 29 patients sharp insturment or gun shot injuries (27.9%), 8 patients traffic accident (7.7%), 1 patient landsıide accident (0.9%),6 patients from the recent greft anastomosis (5.8%), 41 patients A-V fistula that were performed for chronic renal failure (39.4%) and 19 patients during diagnostic angiographic evoluation (18.3%). Cutting the pseudoaneurysmatic segment and restoration of the proximal and distal segments by primary end to end anastomosis or safenous vein interpozition is the basic treatmentapproach. Results: Reason of primer ethiological infection secondary wound infection was developed at 8 cases. At 6 of them the culture was (+) and two of them was gone to debridment for this reason. In 4 patients tissue flebs were used to close the large tissue loss. In all patients the grafts patency were checked with clinic evoluation and Doppler USG. While clinical follow- up, recuren pseudoaneurysm was seen in 2 patients. Conclusion: Meanwhile if the ethiological reason is iatrogenic or traumatic in pseudoaneurysm early definition and early treatment is very important for the patients health. Even if the treatment time delays, it is getting much harder that treatment and complication risks are increasing, also the tissue and/or loss risk is increasing.
