Comparison of the Ortolani' s palpation method, Norberg angle andsubluxation index in the diagnosis of hip joint laxity and hip dysplasia in dogs

Köpeklerde kalça eklemi gevşekliğinin ve bununla ilişkili olarak da kalça displazisine yatkınlığın değerlendirilmesinde, Ortolani palpasyon metodu, Norberg açısı ve sublukzasyon indeksi sonuçlarının etkinliklerinin karşılaştırılmasının amaçlandığı bu çalışmada 8 farklı ırktan 227 köpeğe (Alman Çoban Köpeği, Labrador Retriever, Golden Retriever, Pointer, Mallinois, Doberman Pinscher, İrlanda Seter ve Kangal) ait 454 kalça eklemi, üç metodla ayrı ayrı muayene edildi. Radyografik ölçümler standart ventrodorsal gergin bacak pozisyonunda ve sublukzasyon pozisyonunda çekilen filmler üzerinden alındı. Ortolani'nin palpasyon yöntemiyle muayenesi yapılan kalça eklemlerinin % 71, 8 oranında sağlıklı, % 16,1 hafif pozitif, % 7,3 orta derecede pozitif ve % 4,8 oranında ise şiddetli pozitif olduğu, buna karşın; Norberg açısında % 59,9 sağlıklı, % 26, O hafif pozitif, % 5,3 orta derecede pozitif ve % 8,8 şiddetli pozitif, ve sublukzasyon indeksinde (SI) ise % 35,2 sağlıklı, % 33,50 hafif pozitif, % 20,9 orta derecede pozitif ve % 10,4 oranında ise şiddetli derecede pozitif olduğu belirlendi. Sublukzasyon indekslerinin 0,10 - 0,92 arasında, Norberg açılarının ise 70°- 115° arasında değiştiği belirlendi. Abstract

In this study, which was purposed the comparison of the results of the Ortolani's hip palpation method, Norberg Angle (NA) and Subluxation Index (SI) in order to diagnose of joint laxity and probability of canine hip dysplasia (CHD), 454 hip joints of 227 dogs were evaluated belonging to eight breeds (German Shepherd, Labrador Retriever, Golden Retriever, Pointer, Mallinois, Doberman Pinscher, Irish Setter, Anatolian Karabash) with three methods, separately. Radiographic measurements were taken with femurs extended in standard position (Norberg Angle) and subluxation position. During the examination carried out via Ortolani's palpation method, 71.8% of the hips were evaluated as healthy, 16.1% mild positive, 7.3% moderate positive and 4.8 % severe positive, according to NA method, 59.9% healthy, 26.0% mild positive, 5.3% moderate positive and 8.8% severe positive, and regarding SI 35.2% were healthy, 33.5% mild positive, 20.9% moderate positive and 10.4% severe positive. Subluxation indices ranged 0.10-0.92 and NA results ranged from 70° to 11.5°. There was no significant difference between the results of three methods for rightand left hip joints and male and female dogs. But there was a significant difference between the results of hip joint palpation, NA and SI (p< 0.001). When the results of N A and Ortolani's palpation methods were compared, 93.4% of 272 hips, examined and found to be health by NA, were seemed to be healthy by Ortolani's palpation method. 45% of 40 hips showing severe positive NA results were identified as severe positive in Ortolani's palpation method. When the results of NA and SI methods were compared, 44.8% of 272 hips, examined and found to be health by NA, were seemed to be mild-moderate dysplastic in SI. 35.0% and 65.0% of 40 hips showing severe positive results in NA were identified as moderate and severe dysplastic, respectively with SI. Ortolani's palpation method and NA results were nearly similar in healthy dogs, however, 55% of the severe positive hips via NA, seemed to be mild-moderate severe dysplastic with Ortolani's palpation, and these findings indicated that in severe dysplastic hips, Ortolani's palpation method might give false negative results. In addition, there was a closed relationship between NA and SI results for severe dysplastic hips. However there was an increase in the numbers of the hips, which were healthy with NA but were also mild positive(36.0%) in SI. This study, which was performed to compare the results of the Ortolani's hip palpation, N A and SI in the diagnosis of hip joint laxity and hip dysplasia in dogs, demonstrated that both SI and Ortolani's palpation methods might give false positive (mild-moderate) or false negative results. However faulty results gathered by using SI were found to be in a lesser extent than those gathered by using Ortolani's palpation. As a result, it was recommended that NA and SI should be performed for hip joints to be fully evaluated for dogs suspected of having CHD.
