Özet

Bu araştırma, köpeklerde deneysel oluşturulan iki tip intestinal strangülasyon obstrüksiyon sonucu oluşan işemi-reperfüzyon hasarında; bir hidroksil radikali temizleyici-toplayıcı ajan olan dimetilsülfoksit (DMSO)'in koruyucu etkisinin be-lirlenmesi amacıyla yapıldı. Çalışma materyalini 24 adet köpek oluşturdu. Köpekler kendi aralarında 6'şarlı 4 gruba ayrıldı. Grup l ve ll'deki köpeklerde 1 saat işemi ve 1 saat reperfüzyon gerçekleştirildi ve tedavi amacıyla reperfüzyonun baş-langıcında grup l'e % 0.9'luk NaCI 5 ml/kg dozunda, grup ll'ye de 1 g/kg dozunda DMSO'nun % 20'lik solüsyonu iv uygulandı. Grup III ve IV'teki köpeklerde 24 saat işemi ve 1 saat reperfüzyon gerçekleştirildi ve tedavi amacıyla reperfüzyonun başlangıcında grup IH'e % 0.9'luk NaCI 5 ml/kg dozunda, grup IV'e de 1 g/kg dozunda DMSO'nun % 20'lik solüsyonu iv uygulandı. Grup l ve H'de aperfüzyon oluşturmak amacıyla distal jejunum segmentini besleyen arteria ve vena jejunales'e Buldog klempleri yerleştirildi. Grup III ve IV'te hipoperfüzyon oluşturmak amacıyla aynı damarlara kan akımını tahmini olarak % 50 oranında gerileten ligatürler uygulandı. Bütün gruplardaki köpeklerde klemp ve ligatür uygulanan damarların beslediği 20-30 cm'lik bağırsak segmentinde lüminal oklüzyon oluşturmak amacıyla hem proksimale hem de distale penroz lastik drenlerle ligatürler uygulandı. Bütün gruplardaki köpeklerden laboratuvar analizleri için; anesteziden hemen sonra, (pre-işemi), intestinal strangülasyon obstrüksiyonu oluşturduktan sonra (işemi) ve uygulanan tüm ligatürler kaldırıldıktan ve belirtilen ilaçlar uygulandıktan sonra (reperfüzyon), kan, periton sıvısı ve bağırsak biyopsi örnekleri toplandı. Kan örneklerinde; eritrosit sayısı, hematokrit değer, trombosit sayısı, akyuvar sayısı, hemoglobin miktarı, pH, $PCO_2$, $PO_2$, $HOC_{3^-}$, baz fazlalığı (BE), O2 saturasyonu, Na+, K+, Ca+2, ve glikoz miktarı belirlendi. Periton sıvısında laktik asit seviyesi belirlendi. Abstract

The aim of this study is to evaluate the protective effect of dimethylsulfoxide (DMSO), a scavenger of hydroxyl radical, on the experimentally induced ischemia reperfusion injury by two different intestinal strangulation obstruction in dogs. A total of 24 dogs were used and divided into 4 groups, each group having 6 dogs. The dogs in groups 1 and 2 were subjected to 1 hour of no flow ischemia and 1 hour of reperfusion, and then, for the treatment of animals at the beginning of reperfusion, group 1 was treated with 0.9% NaCI (5 ml/kg), and the other with 20% DMSO (1 g/kg). The dogs in groups 3 and 4 were subjected to 24 hours of low flow ischemia and 1 hour of reperfusion, and then, for the treatment of animals at the beginning of reperfusion, group 3 was treated with 0.9% NaCI (5 ml/kg), and the other with 20% DMSO (1 g/kg). To carry out no flow (aperfusion) ischemia the jejunal artery and vein of the distal jejunum close to ileum were clamped comp-letely with Bulldog clamps in groups 1 and 2. To carry out the low flow ischemia (hipoperfusion) the vessels mentioned above were ligated to decrease blood flow about 50% in groups 3 and 4. The distal jejunal segment was also ligated with penrose drains in its proximal and distal ends 20-30 cm apart for luminal occlusion in all animals. Blood, peritoneal fluid and intestinal biopsy samples were collected both before ischemia and after ischemia and reperfusion. Complete blood count, oxygen status, acid-base, electrolyte and glucose levels in blood were measured, and lactic acid level in peritoneal fluid was also measured.
