Preterm Bebeklerde Non-invaziv Mekanik Ventilasyon Yöntemlerinin Değerlendirilmesi

Özet

Amaç: Non-invaziv ventilasyon, yapay bir hava yolu kurmaya gerek kalmadan ventilasyon desteği sağlar. Pretermlerde non- invazif ventilasyon, invaziv mekanik ventilasyon, sürfaktan replasmanı ve oksijen takviyesi ihtiyacını azaltır. Bu çalışmada çok düşük do ğum a ğırlıklı preterm bebeklerde, do ğum sonras ı non-invaziv ventilasyon uygulamalarının entübasyonu önlemedeki başarısını karşılaştırmayı amaçladık. Gereç ve Yöntem: Ocak 2017-2019 tarihleri aras ında üçüncü düzey yenidoğan yo ğun bak ım ünitesinde izlenen, 32 hafta ve altı gestasyon ya şı olan, do ğum salonunda entübasyon gereksinimi olmayan, respiratuar distres sendromu olan, non invaziv ventilasyon uygulanan hastalar çalışmaya dahil edildi. Hastalar nasal continuous positive airway pressure (NCPAP) ve nasal intermittent positive pressure ventilation (NIPPV) grubu olarak ikiye ayrıldı. Birincil sonuç; postnatal birinci hafta içinde entübasyon gereksinimi olarak belirlendi. Veriler retrospektif olarak değerlendirildi. Bulgular: Çalışmamıza, toplam 134 hasta alındı. Hastaların 85’i (%63.4) NCPAP, 49’u (%36.6) NIPPV moduna al ınmıştı. Demografik veriler incelendiğinde; do ğum a ğırlığı, gestasyon haftası, cinsiyet, doğum şekli aç ısından gruplar aras ında fark yoktu. Non-invaziv ventilasyon ba şarısızlığı NCPAP grubunda %40, NIPPV grubunda %53 olup, gruplar arasında istatistiksel fark saptanmadı (p=0.14). Pnömotoraks oran ı NIPPV grubunda daha yüksek, total mekanik ventilatör süresi daha düşüktü. Orta dönem morbiditelerden patent duktus arteriozus, intraventriküler kana ma, periventriküler lökomalazi, nekrotizan enterokolit oranları açısından fark yoktu. Hastanede kalış süreleri benzerdi. Sonuç: Non-invaziv tekniklerden olan NCPAP ve NIPPV çalışmamızda, primer sonuç olan entübasyonu önlemede birbirinden üstün bulunamamıştır.

Abstract

Objective: Non-invasive ventilation provides ventilation support without the need to estab lish an artifici al airway. Non- invaziv ventilation reduces the need for invasive mechanical ventilation, surfactant replacement and oxygen supplementation in preterms. We aimed to compare the success of postnatal non-invasive ventilation practices to prevent intubation in very low birth weight preterm infants. Material and Method: Patients who were followed in the third level neonatal intensive care unit between January 2017-2019, who had ≤32 weeks gestation years, who did not need intubation in the de livery room, who had respiratory distress syndrome and who had non-invasi ve ventilation were included in the study. Patients were divided into two groups: Nasal Continuous Positive Airway Pressure (NCPAP) and Nasal intermittent positive pressure ventilation (NIPPV). The primary result; postnatal intubation requ irement was determined in the first week. Data were evaluated retrospectively. Results: A total of 134 patients were included in our study. Of the patients, 85 (63.4%) were in NCPAP and 49 (36.6%) were in NIPPV mode. When the demographic data were examined; There were no differences between the groups in terms of birth weight, gestation week, gender, ty pe of delivery. Non-invasive ventilation failure was 40% in the NCPAP group and 53% in the NIPPV group and no statistically significant difference was found between the groups (p = 0.14). The rate of pneumothorax was higher in the NIPPV group and the duration of total mechanical ventilation was lower. There were no significant differences in mid-term morbidities between patent ductus arteriosus, intraventricular hemorrhage, periventricular leukomalacia and necrotizing enterocolitis. The duration of hospitalization was similar. Conclusion: In our NCPAP and NIPPV study, which is one of the non-invasive techniques, the primary result was not superior in preventing intubation.
