Özet

Amaç: Obezite s ıklığı giderek artan, komplikasyonları uzun dönemde ortaya çıkan, morbiditesi ve mortalitesi yüksek ciddi bir hastal ıktır. Bu araştırmada obezitenin komplikasyonlarının erken tan ısında yardımcı olabilecek ve yaygın olarak kullanılabilecek en iyi antropometrik ölçümün bulunması hedeflenmiştir. Yöntem ve gereçler: 01.06.2010-01.12.2010 tarihleri aras ında endokrin polikliniğine obezite nedeniyle ba şvuran ve pubertal evresi 4-5 olan hastalar prospektif olarak çalışmaya alındı. Hastaların vücut kitle indeksi (VKİ), bel çevresi/kalça çevresi (BÇ/KÇ), ya ğ kitle indeksi (YK İ), triponderal kitle indeks (TKİ), a ğırlık/boy (A/B) ve bel çevresi/boy (BÇ/B) oranlar ı hesaplandı ve her bir ölçüm metabolik sendrom ayrımı için değerlendirildi. Bulgular: Yaş ortalaması 14,02±1,62 y ıl olan 41 (%63,1)’i kız, 24 (%36,9)’ ü erkek toplam 65 hasta ara ştırmaya al ındı. Metabolik sendrom varl ığının ayrımında incelenen VKİ, BÇ/KÇ, YKİ, YKİ, A/B oranlarının tan ı performansları ayrı ayrı ROC analizi ile incelendi. Erkeklerde BÇ/KÇ oranı kızlarda BÇ ve BÇ/B oranı anlamlı olarak tespit edildi. Sonuç: Obezitenin komplikasyonlarının erken tanısında yardımcı olabilecek ve yaygın olarak kullanılabilecek en iyi antropometrik ölçüm er keklerde BÇ/KÇ oran ı, k ızlarda ise BÇ ve BÇ/B oranıdır.

Abstract

Objective: Obesity is a serious disease with an increasing frequency, complications occurring in the long term, and high morbidity and mortality. In this study, it was aimed to find the best anthropometric measurement that can help in the early diagnosis of obesity complications and which can be used widely. Methods: The prospective study included patients with pubertal stage 4-5 that presented to Pediatric Endocrinology outpatient clinic due to obesity between 01.06.2010-01.12.2010. Body mass index (BMI), wa ist circumference / hip circumference (WC / HC), fat mass index (FMI), triponderal index (TMI), weight / height (W / H) and waist circumference / height (WC / H) ratios were calculated and each antropometric measurement was evaluated for the distinction of metabolic syndrome Results: The study included a total of 65 patients, comprising 41(63.1%) girls and 24(36.9%) boys with a mean age of 14.02±1.62 years. The diagnostic performances of BMI, WC / HC, FMI, TMI, W / H ratios, which were examined in the distinction of metabolic syndrome, were analyzed separately by ROC analysis. WC / HC ratio in boys, WC and WC / H ratio in girls were significantly determined. Conclusion: The best anthropometric measurement that can help in the early diagnosis of obesity complications and that can be used widely is the WC / HC ratio in boys and the WC and WC / H ratio in girls.
