İdiyopatik Periferik Fasiyal Paralizili Çocuklarda Hematolojik Parametrelerin ve Paralizi Derecesi ile İlişkilerinin Değerlendirilmesi

Özet
Amaç: Bell’s paralizisi (BP), di ğer ad ıyla idiyopatik periferik fasiyal paralizi, başka bir nörolojik bulgunun eşlik etmediği, akut gelişen tek tarafl ı yüz güçsüzlüğünün en sık nedenidir. Etiyopatogenezi halen açıklığa kavuşturumamada, inflamasyonun patogenezde önemli rol oynadığı düşünülmektedir. Nötrofil lenfosit oranı (NLO), trombosit lenfosit oranı (PLO) ve ortalama trombosit hacmi (MPV) son dönemlerde farklı hastalıkların tanı ve takibinde kullanılan inflamasyon belirteçleridir. Bu çalışmada BP’li çocuklarda NLO, PLO, MPV ve eritrosit dağılım genişliği (RDW) kullanılarak inflamatuar etiyolojiyi düşündüren herhangi bir ipucu olup olmadığını araştırmak ve bu belirteçler ile fasiyal paralizi şiddeti arasında ili şki olup olmad ığını belirlemek amaçlandı. Gereç ve Yöntemler: Bu retrospektif çalışma, Ocak 2018 ile Ocak 2020 yılları arasında, BP tan ısı alan 25 çocuk hasta ve rastgele seçilmiş 25 sağlıklı kontrol grubu ile yapıldı. İki grup nötrofil say ısı, lenfosit say ısı, NLO, PLO, MPV ve RDW seviyeleri açısından karşılaştırıldı. Ayrıca hasta grubunda inflamatuar belirteçlerin paralizisi derecesine göre karşılaştırması yapıldı. Bulgular: Hasta grubunda ortalama nötrofil, lökosit sayısı ve NLO de ğeri kontrol grubuna göre istatistiksel olarak anlamlı derecede yüksekti (sırasıyla; p = 0,001, p = 0,016). Hasta grubunda NLO yüksekliği ile paralizinin şiddeti aras ında anlaml ı bir ilişki bulunmadı. Ortalama lenfosit sayısı, PLO, MPV ve RDW değerleri bakımından iki grup arasında istatistiksel olarak anlamlı fark gösterilemedi. Sonuç: Bell’s paralizili çocuklarda NLO ve nötrofil say ısındaki artış, bu hastalığın etiyopatogenezinden sorumlu oldu ğu düşünülen inflamatuar süreci desteklemektedir. Bu nedenle, bu parametreler idiyopa tik periferik yüz felci olan pediatrik hastalarda tan ıyı desteklemek, ayırıcı tanıya yardımcı olmak veya tedaviye rehberlik etmek için kullanılabilir.
Abstract
Objective: Bell's palsy, also known as idiopathic peripheral facial paralysis, is the most common cause of unilateral facial weakness that is not accompanie d by any other neurological finding. Although the pathogenesis of Bell palsy is not fully understood, inflammation is t hought to play an important role. Neutrophil to lymphocyte ratio (NLR), platelet/lymphocyte ratio (PLR), and mean mean platelet volume (MPV), are among the markers used to determination of inflammation. The study aimed to investigate the presence of any clues suggesting inflammatory etiology using NLR, PLR, MPV, and red cell distribution width (RDW) in children with Bell's palsy and to determine whether there was any relationship between these markers and the degree of paralysis. Materials and Methods: This retrospective study was conducted between January 2018 and January 2020 with 25 children with Bell's palsy and 25 randomly selected healthy controls. Two groups were compared in terms of neutrophil count, NLR, PLR, MPV, and RDW levels. Results: Mean neutrophil, leukocyte counts, and NLR were statistically significantly higher in patients with Bell Palsy than in the control group (p=0 .001, p = 0.016, respectively). However, no significant relationship was found between the NLR elevation and the severity of facial paralysis in the patient group. There was no statistically significant difference between the two groups in terms of the mean of lymphocyte count, PLR, MPV, and RDW values. Conclusions: The increase in NLR and neutrophil count in children with Bell palsy suppor ts the inflammatory process thought to be responsible for the etiopathogenesis of this disease. Therefore, these parameters can be used to support the diagnosis, assist in the differential diagnosis, or guide treatment in pediatric patients with peripheral facial palsy.
