Yoğun Bakım ve Serviste Takip Edilen Pcr Pozitif Covid 19 Tanılı Hastaların Genel Özelliklerinin Karşılaştırılması

Özet
Amaç: 8 Aralık 2019’da Çin’in Hubei eyaletine bağlı Wuhan şehrinde nedeni bilinmeyen pnömoni vakaları rapor edildi ve Dünya Sağlık Örgütü (DSÖ), 7 Ocak 2020’de pnömoni etkeninin insanlarda infeksiyona neden olan yeni bir koronavirüs olduğunu ortaya koymuştur. Bu virüs, DSÖ tarafından 2019-nCoV (2019-novel coronavirus), neden olduğu hastalık ise COVID-19 (coronavirus disease-2019) olarak tanımlanmıştır. Amacımız, yoğun bakım ve serviste yatan covid 19 kesin tanılı hastaların genel özelliklerini sunmak ve bu hastalara ait tedavi deneyimlerimizi paylaşmaktır. Yöntem: 17 Mart 2020 - 1 Mayıs 2020 tarihleri arasında hastanede yatırılarak tedavi edilen, PCR Covid 19 test sonucu pozitif olan 103 hasta retrospektif olarak incelendi. Bulgular: Hastaların ortanca yaşı 42 (28-58) idi ve %58,3’ü erkekti. Yoğun bakımda takip edilen hastalarda hipertansiyon (HT) ve diyabetes mellitus (DM) sıklığı ile radyolojik olarak tipik görünüm oranı serviste takip edilen hastalara göre anlamlı olarak daha fazlaydı. En sık görülen semptomlar öksürük, ateş, nefes darlığı ve boğaz ağrısıydı. Yoğun bakımda yatan hastalarda lenfosit ve albümin düzeyleri anlamlı olarak daha düşük iken; C- reaktif protein (CRP), D-dimer, Ferritin, laktat dehidrogenaz (LDH), nötrofil lenfosit oranı (NLO), platelet lenfosit oranı (PLO) ve C-reaktif protein albumin oranı (CAR) düzeyleri anlamlı olarak daha yüksekti. Yoğun bakıma yatan hastalarda tocilizumab tedavi uygulanma oranı, serviste yatan hastalara göre anlamlı olarak daha yüksekti. Sonuç: Bu çalışmada, yoğun bakım gereksinimi olan hastaların daha yaşlı, BT’de tipik radyolojik görünümü daha sık, komorbid durumlar daha fazla, inflamatuvar parametrelerin daha yüksek olduğunu ve kombine tedavi gereksinimini tespit ettik.
Abstract
Objective: On December 8, 2019, pneumonia cases of unknown cause were reported in Wuhan, China's Hubei province, and the World Health Organization (WHO) revealed on January 7, 2020 that the cause of pneumonia is a new coronavirus that causes infection in humans. This virus has been defined by WHO as 2019-nCoV (2019-novel coronavirus) and the disease it causes as COVID-19 (coronavirus disease-2019). Our aim is to present the general characteristics of covid 19 patients with definite diagnosis in intensive care and service and to share our treatment experiences of these patients. Metods: 103 patients with positive PCR Covid 19 test results who were hospitalized between March 17, 2020 and May 1, 2020 were retrospectively analyzed. Results: The median age of the patients was 42 (28-58) and 58.3% of them were male. The frequency of HT and DM and the typical radiological appearance rate were significantly higher in patients followed up in the intensive care unit compared to the patients followed in the service. The most common symptoms were cough, fever, shortness of breath and sore throat. While lymphocyte and albumin levels were significantly lower in intensive care patients; C- reactive protein (CRP), D-dimer, Ferritin, lactate dehydrogenase (LDH), neutrophil lymphocyte ratio (NLR), platelet lymphocyte ratio (PLR) ve C-reactive protein albumin ratio (CAR) levels were significantly higher. The rate of Tocilizumab treatment in intensive care patients was significantly higher than in patients hospitalized in the service. Conclusion: In this study, we determined that patients in need of intensive care were older, typical radiological appearance on CT was more frequent, comorbid conditions were higher, inflammatory parameters were higher, and combined treatment requirements were found.
