Yeni Tanı Multiple Myeloma’lı hastalarda Lenalidomid İdame Tedavisinin Prognostik Değeri; Tek Merkez Deneyimi-Geriye Dönük Çalışma

Özet
Amaç: Multiple myeloma (MM) en yaygın görülen malign plazma hücre hastalığıdır. Son 10 yılda tedavideki gelişmeler daha uzun genel sağkalım ve ilerlemesiz sağ kalım sürelerine ulaşılmasını sağlamıştır. Tedavideki bu ilerlemelere rağmen otolog kök hücre nakli (OKHN) sonrasında da tedavisiz izlenen hastalar nüks etmektedir. Nüksü geciktirmek için idame tedavi stratejileri gündeme gelmiştir. Merkezimizde lenalidomid idame tedavisi uyguladığımız yeni tanı MM’lı hastaların sonuçlarını paylaşmak istedik. Gereç ve Yöntemler: Ocak 2018-0cak 2020 tarihleri arasında MM tanısı ile OKHN yapılan 22 hasta ve OKHN’e elverişli olmayıp indüksiyon tedavisi sonrası lenalidomid idamesi başlanan üç hasta çalışmaya dahil edildi. Bulgular: Hastaların beşinde (%20) nötropeni, üçünde (%12) trombositopeni, dokuzunda (%36) pnömoni gelişti. Hastaların sekizinde (%32) enfeksiyon, hastalık progresyonu ve toksisite nedeni ile idame tedavisi kesildi. Lenalidomid kesilen hastalara pomalidomid, karfilzomib veya ixazomib içeren dördüncü basamak tedavileri başlandı. Son durumda bir hasta (%4) progresif hastalık nedeniyle vefat etti. Bir hasta stabil hastalık, altı hasta tam yanıt, altı hasta kısmi yanıt, 11 hasta ise çok iyi kısmi yanıt mevcuttu. Hastalar en az iki ay en çok 44 ay (ortanca 12 ay) lenalidomid idamesi kullandılar. Sonuç: Bu sonuçlar MM’lı hastalarda hem genel sağkalım hem de ilerlemesiz sağkalım oranlarını artırmak için idame tedavisinin rolünü desteklemektedir.
Abstract
Objective: Multiple Myeloma (MM) is the most common malign plasma cell disease. The developments in the treatment, in the last 10 years, have enabled longer survival. Despite these advances in treatment, patients who received no maintenance treatment relapsed, after autologous stem cell transplantation (ASCT). Maintenance treatment strategies have been introduced to delay relapse. We wanted to share the results of patients who were NDMM, for whom we received lenalidomide maintenance treatment, in our center. Materyal and methods: Between January 2018-January 2020, 22 patients were included in the study, who were diagnosed with MM and received ASCT, and were ineligible for transplantation, started lenalidomide maintenance treatment after induction treatment. Results: Five of the patients developed neutropenia (20%), three of them developed thrombocytopenia (12%), and nine of them developed pneumonia (36%). The maintenance treatment was discontinued in eight of the patients (32%), due to infection, progression of the disease and toxicity. Fourth line treatments was started in patients who discontinued lenalidomide. In the end, one patient died due to the progressive disease (4%). It was followed that one patient was stable disease, six were complete remission, the other six were partial remission, and 11 patients were very good partial remission to the treatment. Patients received lenalidomide maintenance treatment from two months the least, and 44 months (median 12 months) the most. Conclusion: These results support the role of maintenance therapy to increase both overall survival and progression-free survival rates in patients with MM.
