Pnömonili Olgularda Akciğer Ultrasonografisinin Prognoz Üzerine Etkisi

Özet

Amaç: Çalışmamızda pnömoni ön tanısı ile Göğüs Hastal ıkları Kliniğinde ve Anestezi Yoğun Bakım Ünitesinde takip edilen olgularda akciğer ultrasonografisinin prognostik faktörlere (parapnömonik efüzyon, ampiyem geli şimi vs.) etkisini incelemeyi amaçladık. Gereç ve Yöntem: Çalışmamıza 01.01.2017 - 01.06.2018 tarihleri aras ında Van Yüzüncü Y ıl Üniversitesi Tıp Fakültesi Göğüs Hastal ıkları Kliniğinde ve Anestezi Yoğun Bak ım Ünitesinde pnömoni ve/ veya plevral efüzyon ön tanısı ile takip edilen 50 hasta dâhil edildi. Hastalar akciğer ultrasonografisi ile takip edilerek, bu olguların prognozunu etki leyen faktörler incelendi. Bulgular: Çalışmaya dâhil edilen hastaların 16’s ı kadın (%32), 34’ü erkek (%78); yaş ortalamaları 60.4±19.4 (min. 15- max. 87) idi. Plevral efüzyon olguların %56’s ında saptand ı. Torasentez yapılan hastalarda Light kriterleri değerlendirildiğinde ortalama LDH ( 593.89±832.6), glukoz (102.50±46.67 mg/dl); albümin (2.93±3.46g/dl) olarak saptandı. 28 olgunun %28,57’si transuda, %71,42’si eksuda olarak de ğerlendirildi. Torasentez yapılan 28 olgunun s ıvı hemogram analizlerinde % 39,3 ‘ünde lenfosit, % 46,4’ünde nötrofil, % 14,3 ‘ünde eosinofil hâkimiyeti mevcut idi. Tüberküloz ön tan ısı nedeni ile 4 olguda bakılan ADA düzeyi ortalama 48,75 ±2,5 IU/L (min. 45- max 50) olarak saptandı. 4 olguya tüp torakoskopi, 1 olguya dekortikasyon yapıldı, fibrinolitik ge reken hasta olmad ı. Olguların ortalama yatış süresi 7.78±5.44 gün (min. 2-Max 22) idi. Sonuç: Çalışmamızda ortaya çıkan veriler ne ticesinde akciğer ultrasonografisinin, giri şimsel işlemlerde yüksek başarısı, komplikasyonun az oluşu ve en önemlisi başta yo ğun bak ım olmak üzere hasta ba şında işlem imkânı sunması gibi avantajları nedeniyle güncel pratikte daha sık kullanılması gerekliliğini desteklediği kanaatindeyiz.

Abstract

Objective: In our study, we aimed to examine the effect of Lung ultrasonography on progno stic factors (parapneumonic effusion, empyema development, etc.) in patients with a diagnosis of pneumonia who were followed up in the Chest Diseases Clinic and Anesthesia Intensive Care Unit. Material and Method: Fifty patients with a pre-diagnosis of pneumonia and / or pleu ral effusion were included in our study between 01.01.2017 - 01.06.2018 in the Chest Diseases Department of Van Yüzüncü Yıl University Medical Faculty and in the Anesthesia Intensive Care Unit. The patients were followed up with lung ultrasonography and the factors affecting the prognosis of these cases were examined. Results: Of the patients included in the study, 16 were female (32%), 34 were male (78%); Their mean age was 60.4 ± 19.4 (15-87). Pleural effusion was detected in 56% of the cases. When the Light criteria were evaluated in patients who underwent thoracentesis, mean LDH (593.89 ± 832.6), glucose (102.50 ± 46.67 mg / dl); albumin (2.93 ± 3.46g / dl). 28.57% of 28 cases were evaluated as transudate and 71.42% as exudate. In the fluid hemogram analysis of 28 patients who underwent thoracentesis, 39.3% of lymphocytes, 46.4% of neutrophils, 14.3% of eosinophils were domi nant. Due to the pre-diagnosis of tuberculosis, the average ADA level measured in 4 cases was found to be 48.75 ± 2.5 IU/L (min. 45- max 50). Tube thoracoscopy was performed in 4 cases and decortication in 1 case, no patient required fibrinol ytic. The mean hospitalization period of the patients was 7.78 ± 5.44 days (min. 2-Max 22). Conclusion: As a result of the data obtained in our study, we are of the opinion that lung ultrasonography supports the need for more frequent use in current practice due to its advantages such as high success in interventional procedures, low complication rate, and most importantly, the opportunity to operate at the bedside, especially in intensive care.
