Yenidoğan Sepsisi Tanısı ve Prognozunda IL-6, IL-8, TNF-α ve C-Reaktif Protein Düzeyleri

Özet
Amaç: Çalışmada, IL-6, IL8, TNF-α ve C-reaktif protein (CRP) düzeylerinin yenidoğan sepsisinde tanı ve prognoz açısından önemi ve güvenirliğinin saptanması amaçlandı. Gereç ve Yöntem : Yenidoğan yoğunbakım ünitesinde yenidoğan sepsisi ön tanısıyla takip ve tedavisi yapılan 30 yenidoğan ve herhangi bir hastalığı olmayan annelerden problemsiz olarak doğan sağlıklı 20 yenidoğan çalışmaya dahil edildi. Olguların cinsiyet, gestasyonal yaş, postnatal yaş, doğum yeri ve şekli, doğum ağırlığı, IL-6, IL-8, TNF-α ve CRP seviyeleri kaydedildi. Bulgular: Sepsis tanısı konulan 30 olgunun 16 (% 53.3)’ü erkek, 14 (% 46.7)’si kız idi. Kontrol grubundaki 20 vakanın ise 11 (% 55)’i erkek, 9 (% 45)’u kız idi. Sepsis tanılı vakaların 8’i erken başlangıçlı (% 26.6), 22’si geç başlangıçlı (% 73.4) yenidoğan sepsisi olarak kabul edildi. Erken başlangıçlı sepsiste mortalite oranı % 25 iken, geç başlangıçlı sepsis olgularında bu oran % 36.3 idi. Sepsisli olguların olduğu grupta, C-reaktif protein, İnterlökin-6 ve İnterlökin-8, kontrol grubundan istatistik olarak anlamlı derecede daha yüksek bulundu. Tümör nekrozis faktör-α düzeyleri bakımından gruplar arası fark istatistik olarak anlamlı bulunmadı. Eksitus olan olgularda IL-6 (p: 0.001) ve IL-8 (p: 0.007) düzeyleri istatistik olarak kontrol grubuna göre anlamlı derecede yüksek bulundu. Sonuç: Yenidoğan sepsisi tanısında C-reaktif protein, İnterlökin-6, İnterlökin-8 düzeyleri kullanılabilecek parametreler olarak bulunurken, TNF-α tanısal değere sahip bulunmadı. Yenidoğan sepsisi prognozunda ise IL-6 ve IL-8 düzeyleri anlamlı bulundu.
Abstract
Introduction: The aim of the study was to determine the importance and reliability of IL-6, IL8, TNF-α and C-reactive protein (CRP) levels in terms of diagnosis and prognosis in neonatal sepsis. Materials and Methods: Thirty newborns who were followed up and treated with the prediagnosis of neonatal sepsis in Neonatal Intensive Care Unit and 20 healthy newborns born to mothers without any disease were included in the study. Gender, gestational age, postnatal age, place and type of birth, birth weight, IL-6, IL-8, TNF-α and CRP levels of the cases were recorded. Results: Of the 30 cases diagnosed clinically as sepsis, 16 (53.3 %) were male and 14 (46.7 %) were female. Of the 20 cases in the control group, 11 (55 %) were male and 9 (45 %) were female. Eight (26.6 %) of 30 sepsis cases were accepted as earlyonset and 22 (73.4 %) late-onset neonatal sepsis While the mortality rate in early-onset sepsis was 25 %, this rate was 36.3% in late-onset sepsis cases. C-reactive protein, Interleukin- 6, and Interleukin-8 were found to be statistically significantly higher in the group of cases than in the control group. Tumor necrosis factor-α levels were statistically similar in both groups. IL-6 (p: 0.001) and IL-8 (p: 0.007) levels were found to be statistically significantly higher in deceased cases. Conclusion: C-reactive protein, Interleukin-6, Interleukin-8 levels in the diagnosis of neonatal sepsis found as parameters that can be used, TNF-α was not found to have diagnostic value. IL-6 and IL-8 levels were found to be significant in neonatal sepsis prognosis.
