Özet
Giriş: Metastatik lenf nodu oran ının (mLNO) baz ı solid organ tümörlerinde önemli bir prognostik faktör olduğu gösterilmiştir. Bu çalışmanın amac ı mLNO’nun kolorektal kanserli (KRK) hastalardaki prognostik önemini araştırmaktır. Yöntem: Ocak 2008- Nisan 2014 tarihleri arasında, uzak metastaz ı olmayan ve küratif amaçla cerrahi tedavi uygulanan 246 KRK’li hastanın kayıtları retrospektif olarak incelendi. Yaş, cinsiyet, ç ıkarılan toplam lenf nodu sayısı, mLN say ısı, patolojik T evresi, TNM evresi, mLNO ve lenfovasküler invazyonun hastalıksız ve genel sa ğkalım üzerindeki etkisi tek ve çok değişkenli Cox-regression testi ile analiz edildi. Sağ kalım eğrileri Kaplan-Meier, sağ kalım farkları log-rank testi ile hesapland ı. mLNO için optimal cut-off değeri ROC analizi ile belirlendi. Bulgular: Hastaların ortanca yaşı 61 (23-86) olup, ortanca takip süresi 38 (11-78) aydı. ROC analizinde mLNO için optimal cut-off değeri 0.08 olarak bulundu. Abstract
Intoduction: Metastatic lymph node ratio (mLNR) has been shown to be an important prognostic factor in some solid tumours. The aim of this study was to investigate the prognostic value of mLNR in patients with colorectal carcinoma (CRC). Methods: The records of 246 CRC patients who had not distant metastasis and treated with curative surgery were retrospectively evaluated between January 2008 and April 2014. The effects of age, gender, the number of total lym ph nodes removed, the number of metastatic lymph nodes, pathological T stage, TNM staging, mLNR and lymphovascular invasion on di sease free survival and overall survival were analyzed with univariate and multivariate Cox-regression tests. Survival curves were calculated using Kaplan-Meier method and survival differences were assessed by log-rank test. Optimal cut-off value for mLNR is determined with ROC analysis. Results: Median age was 61 (23-86) and median follow-up period was 38 (11-78) months. Optimal cut-off value for mLNR was found as 0.08 in ROC analysis.
