Kilitli Humerus Plağı ile Osteosentez Uyguladığımız 60 Yaş Üstü Proksimal Humerus Kırıklı Hastalarımızın Sonuçları

Özet
Amaç: 60 yaş üstü hastalarda proksimal humerus k ırığının (PHK) tedavisi konusunda tart ışmalar vardır. Özellikle parçalı ve ileri deplasmanlı kırıklar için cerrahi düşünülmelidir. İleri yaştan dolayı oluşan osteoporoz gibi faktörlerden dolay ı, kemik tutulumu iyi olan kilitli proksimal humerus plaklar ı tercih edilmektedir. Çalışmamızda 60 yaş üstü PHK’ları için uyguladığımız kilitli proksimal humerus plaklarının sonuçlarını inceledik. Gereç ve Yöntem: 60 yaş üstü PHK’lı hastalar çal ışmaya alınmıştır. Hastalara, deltopektoral insizyonla aç ık redüksiyon ve kilitli proksimal humerus plağı ile osteosentez uyguland ı. Hastalar radyolojik ve klinik olarak en az 1 yıl takip edilmiştir. 1.yıldan sonra, hastalarımız Constant-Murley omuz skorlamasına (CMOS) göre de ğerlendirilmiştir ve komplikasyon takipleri yapılmıştır. Bulgular: Çalışmamıza PHK’lı tarafımızdan opere edilmiş 31 hasta alınmıştır (25 kadın, 6 erkek). Yaş ortalaması 71.22 (62-87) dir. Hastalarımızın; Neer s ınıflamasına göre 1’i 1 parçalı, 13’ü 2 parçalı, 12’si 3 parçalı, 5’i 4 parçalı ve 3’ü kırıklı çıkıklıdır. Ortalama takip süresi:13.9 (12-18) ay. Ortalama kaynama süresi:13.55 hafta (12-20). Ortalama CMOS 49.74 (23-98) olarak bulunmuştur. 70 ya ş üstü ve altı olan hastaların CMOS’ları arasında anlamlı bir fark bulunmamıştır. Neer tip 1,2 ve tip 3,4 hastaların CMOS’ları arasında anlamlı bir fark bulunmamıştır. Sonuç: 60 yaş üstü PHK’lı hastalarda kilitli humerus plağı uygulamasının komplikasyona açık ve fonksiyonel sonuçlar ı zayıf olduğu için bu hastaların alternatif tedavi seçenekleri de gözönünde bulundurulup ameliyatlar ın ona göre planlanmasının uygun olacağı kanaatindeyiz.
Abstract
Introduction: There is controversy about the treatment of proximal humeral fracture (PHF ) in elderly patients. Surgery should be considered especially for comminuted and advanced displacement fractures. Due to factors such as osteoporosis caused by advanced age, locked proximal humeral plates (LPHP) with good bone invo lvement are preferred. We examined the results of the LPHP that we applied for PHF over 60 years of age. Materials and Methods: Patients with PHF over the age of 60 were included in the study. Patients underwent open reduction with a deltopectoral incision an d osteosynthesis with a LPHP. Patients are followed up for at least 1 year radiologically and clinically. After the first year , our patients were evaluated according to the Constant-Murley shoulder score (CMSS) and complications were followed up. Results: 31 patients with PHF who were operated on by us were included in our study (25 females, 6 males). The mean age is 71.22 (62-87). Our patients; According to the Neer classification, 1 has 1-part, 13 has 2-part, 12 has 3-part, 5 has 4- part and 3 has fracture-dislocati ons. Mean follow-up time: 13.9 (12-18) months. Average union time: 13.55 weeks (12-20). The mean CMSS was found to be 49.74 (23-98). There was no significant difference between the CMSS of patients aged above and below 70 years. No sign ificant difference was found between the CMSS of Neer type 1,2 and type 3,4 patients. Conclusion: Since locked humeral plate application in patients with PCC over 60 years of age is prone to complications and poor functional results, we think that it would be appropriate to plan the surgeries accordingly by considerin g alternative treatment options for these patients.
